喉咽清口服液联合开喉剑喷雾剂治疗小儿疱疹性咽峡炎的临床疗效及其安全性

摘　　要：目的探讨喉咽清口服液联合开喉剑喷雾剂治疗小儿疱疹性咽峡炎的临床疗效及其安全性。方法选取济南市儿童医院急诊科2018年6月—2020年6月收治的疱疹性咽峡炎患儿300例,随机分为喉咽清口服液组(A组)101例、开喉剑喷雾剂组(B组)99例、喉咽清口服液联合开喉剑喷雾剂组(C组)100例。A组给予喉咽清口服液,B组给予开喉剑喷雾剂(儿童型),C组给予喉咽清口服液联合开喉剑喷雾剂。比较3组临床疗效,症状(发热、疱疹、流涎)消失时间及住院时间,治疗前后血清白介素6(IL-6)、C反应蛋白(CRP)、肿瘤坏死因子α(TNF-α),CD_(4)^(+)细胞分数、CD_(4)^(+)/CD_(8)^(+)细胞比值表达水平,并比较3组患儿不良反应发生率。结果C组总有效率高于A组、B组(P<0.05)。3组发热、疱疹、流涎消失时间及住院时间比较,差异有统计学意义(P<0.05);C组发热、疱疹、流涎消失时间及住院时间短于A组和B组(P<0.05)。治疗前3组IL-6、CRP、TNF-α水平比较,差异无统计学意义(P>0.05);治疗后C组IL-6、CRP、TNF-α水平低于A组、B组(P<0.05)。治疗前3组CD_(4)^(+)细胞分数、CD_(4)^(+)/CD_(8)^(+)细胞比值比较,差异无统计学意义(P>0.05);治疗后C组CD_(4)^(+)细胞分数、CD_(4)^(+)/CD_(8)^(+)细胞比值高于A组、B组(P<0.05)。3组不良反应发生率比较,差异无统计学意义(P>0.05)。结论喉咽清口服液联合开喉剑喷雾剂治疗小儿疱疹性咽峡炎的临床疗效确切,能够有效降低炎性因子,改善免疫功能,缩短症状消失时间,且安全性较高。Objective To investigate the clinical efficacy and safety of Houyanqing oral liquid combined with Kaihoujian spray in the treatment of herpetic angina in children.Methods A total of 300 cases of children with herpetic angina were selected from June 2018 to June 2020 in Jinan Children's Hospital,which were randomly divided into the Houyanqing oral liquid group(group A)101 cases,the Kaihoujian spray group(group B)99 cases,and Houyanqing oral liquid combined with the Kaihoujian spray group(group C)100 cases.Group A was given Houyanqing oral liquid,group B was given Kaihoujian spray(child type),group C was given Houyanqing oral liquid combined with Kaihoujian spray.The clinical efficacy,disappearance time of symptoms(fever,herpes,salivation)and hospital stay,serum interleukin-6(IL-6),C-reactive protein(CRP)and tumor necrosis factorα(TNF-α),percentage of CD_(4)^(+)cells and the ratio of CD_(4)^(+)/CD_(8)^(+)cells before and after treatment were compared arnong the three groups.The incidence of adverse reactions was compared among the three groups.Results The total effective rate of group C was higher than that of group A and group B(P<0.05).There were significant differences in fever,herpes,salivation disappearance time and hospital stay among the three groups(P<0.05);the disappearance time of fever,herpes,salivation and hospital stay in group C were shorter than those in group A and group B(P<0.05).The levels of IL-6,CRP and TNF-αin three groups before treatment were not significantly different among the three groups(P>0.05);after treatment,the levels of IL-6,CRP and TNF-αin group C were significantly lower than those in group A and group B(P<0.05).There was no significant difference in CD_(4)^(+)cell fraction and CD_(4)^(+)/CD_(8)^(+)cell ratio among the three groups before treatment(P>0.05);after treatment,the percentage of CD_(4)^(+)cells and the ratio of CD_(4)^(+)/CD_(8)^(+)cells in group C were higher than those in group A and group B(P<0.05).There was no significant difference in the incidence of adverse r
