体重指数与多囊卵巢综合征临床指标的相关性研究

摘　　要：目的探讨不同体重指数(BMI)的多囊卵巢综合征(PCOS)患者的临床特征及BMI与内分泌、糖脂代谢之间的相关性分析。方法从临床科研信息共享系统内提取2016年12月~2019年12月980例PCOS患者的临床体征及生化指标信息。根据BMI将患者分为3组,即18.5kg/m^(2)<BMI<24kg/m^(2)组、24kg/m^(2)≤BMI<28kg/m^(2)组、BMI≥28kg/m^(2)组,比较3组间临床指标的差异,分析临床指标与BMI的相关性。结果PCOS患者中超重和肥胖的发生率分别为33.67%和28.88%,随着BMI的增加,IR的发生率逐渐升高;BMI与DBP、SBP、HOMA-β、IR、FINS、WHR、HC和WC间呈正相关,r值在0.3~0.9之间;与LH和ISI间呈负相关,r值在-0.3~-0.6之间。结论肥胖的PCOS患者更易出现内分泌紊乱和糖脂代谢障碍,随着BMI的增加,内分泌和糖脂代谢紊乱逐渐加重。Objective To reveal the correlation between body mass index and clinical indicators in patients with polycystic ovary syndrome through comparing the patients′clinical and biochemical characteristics.Methods From December 2016 to December 2019,we selected 1074 patients with PCOS and extracted the clinical and biochemical characteristics from the clinical scientific research information sharing system.The patients were divided into three groups of 18.5kg/m^(2)<BMI<24kg/m^(2) group,24kg/m^(2)≤BMI<28kg/m^(2) group,BMI≥28kg/m^(2) group and compared the differences in clinical and biochemical characteristics.Results The inci dence of overweight and obesity in PCOS patients was 33.67%and 28.88%,respectively.With the increase of BMI,the incidence of insulin resistance in PCOS gradually increased.DBP,SBP,HOMA-β,IR,FINS,WHR,HC and WC were positively correlated with BMI,and the range of r was between 0.3 and 0.9.LH and ISI were negatively correlated with BMI,and the range of r was between-0.3 and-0.6.Conclusion The obese patients with PCOS are more prone to endocrine disorders and abnormal glucolipid metabolism.With the gradual increase of BMI,patients with PCOS are gradually aggravated in endocrine and glucolipid metabolism disorders.
