重灸治疗变应性鼻炎的疗效观察及对IL-33、EOS的影响

摘　　要：目的观察穴位重灸治疗常年变应性鼻炎的临床疗效及对白介素(IL)-33水平、嗜酸性粒细胞(EOS)计数的影响。方法将180例常年变应性鼻炎患者随机分为观察组和对照组,每组90例。观察组采用穴位重灸治疗,对照组采用布地奈德鼻喷剂治疗。观察两组治疗前后临床症状评分、鼻结膜炎生存质量量表(RQLQ)评分、血清IL-33水平与EOS计数变化,并比较两组临床疗效、随访20周发作次数和平均程度评分及不良反应发生情况。结果两组治疗后临床症状评分、RQLQ评分显著低于治疗前(P<0.05),观察组低于对照组(P<0.05)。治疗后及随访20周时,两组IL-33表达水平和EOS计数均显著低于治疗前(P<0.05),观察组低于对照组(P<0.05);两组患者随访20周时IL-33表达水平和EOS计数均显著低于治疗后(P<0.05)。观察组总有效率(91.1%)显著高于对照组(78.9%)(P<0.05)。观察组随访20周发作次数和平均程度评分显著低于对照组(P<0.05)。结论穴位重灸治疗常年变应性鼻炎可明显改善患者的临床症状,提高生活质量,减少患者的发作次数,改善IL-33与EOS水平。Objective To observe the clinical efficacy of large-dose moxibustion in treating perennial allergic rhinitis(AR)and its effects on the interleukin(IL)-33 level and eosinophil(EOS)count.Method A total of 180 patients with perennial AR were randomized into an observation group and a control group,with 90 cases in each group.The observation group was treated with large-dose moxibustion at acupoints,while the control group was intervened by Budesonide nasal spray.Clinical symptoms score,the score of Rhinoconjunctivitis quality of life questionnaire(RQLQ),serum IL-33 level,and EOS count were observed in each group.Clinical efficacy was also compared.Attack frequency,average severity score and adverse reactions were compared at the 20-week follow-up.Result Clinical symptoms core and RQLQ score dropped significantly after treatment in both groups(P<0.05),and were lower in the observation group than in the control group(P<0.05).After treatment and at the 20-week follow-up,the IL-33 level and EOS count declined compared with the baseline in each group(P<0.05),and were lower in the observation group than in the control group(P<0.05);at the 20-week follow-up,the IL-33 level and EOS count were lower than those after treatment in each group(P<0.05).The total effective in the observation group(91.1%)was significantly higher than that in the control group(78.9%)(P<0.05).The attack frequency and average severity score were lower in the observation group than in the control group at the 20-week follow-up(P<0.05).Conclusion Large-dose moxibustion at acupoints can notably improve the clinical symptoms,enhance quality of life,reduce attack frequency,and improve the levels of IL-33 and EOS in treating perennial AR.
