火针配合药物治疗神经性皮炎疗效观察和对血清炎症因子表达的影响

摘　　要：目的观察火针配合药物治疗神经性皮炎的临床疗效及其对血清炎症因子表达和患者焦虑抑郁状况的影响。方法将132例神经性皮炎患者随机分为治疗组和对照组,每组66例。治疗组采用火针配合口服地氯雷他定片治疗,对照组采用单纯口服地氯雷他定片治疗。观察两组治疗前后中医症状积分、汉密顿焦虑量表(HAMA)评分、汉密顿抑郁量表(HAMD)评分、皮肤病生活质量指数(DLQI)评分及各项血清炎症因子水平[肿瘤坏死因子(TNF)-α、白细胞介素(IL)-6、IL-8]、生化指标[P物质(SP)、脑脊液β-内啡肽(β-EP)、5-羟基色胺(5-HT)、组胺]的变化情况,并比较两组临床疗效。结果两组治疗后中医症状积分、HAMA评分、HAMD评分、DLQI评分及各项血清炎症因子水平和生化指标均较同组治疗前显著降低(P<0.05)。治疗组治疗后中医症状积分、HAMA评分、HAMD评分、DLQI评分及各项血清炎症因子水平和生化指标均明显低于对照组,两组比较差异具有统计学意义(P<0.05)。治疗组总有效率为97.0%,明显高于对照组的84.9%,两组比较差异具有统计学意义(P<0.05)。结论火针配合药物治疗神经性皮炎疗效确切,能有效降低炎症反应,减轻患者抑郁及焦虑症状。Objective To investigate the clinical efficacy of fire needling plus medication for neurodermatitis and its effect on the expressions of serum inflammatory factors and the patients’anxiety and depression.Method One hundred and thirty-two patients with neurodermatitis were randomized to treatment and control groups,with 66 cases in each group.The treatment group received fire needling plus oral administration of Desloratadine tablets and the control group received oral administration of Desloratadine tablets alone.The TCM symptom score,the Hamilton anxiety scale(HAMA)score,the Hamilton depression scale(HAMD)score and the dermatology life quality index(DLQI)score were recorded,and the levels of serum inflammatory factors[tumor necrosis factor(TNF)-α,interleukin(IL)-6 and IL-8]and biochemical indicators[substance P(SP),andβ-endorphin(β-EP),5-hydroxytryptamine(5-HT)and histamine in cerebrospinal fluid]were measured in the two groups before and after treatment.The clinical therapeutic effects were compared between the two groups.Result The TCM symptom score,the HAMA score,the HAMD score,the DLQI score,the levels of serum inflammatory factors and biochemical indicators decreased significantly in the two groups after treatment compared with before(P<0.05).After treatment,the TCM symptom score,the HAMA score,the HAMD score,the DLQI score,the levels of serum inflammatory factors and biochemical indicators were significantly lower in the treatment group than in the control group with statistically significant differences between the two groups(P<0.05).The total efficacy rate was 97.0%in the treatment group,which was significantly higher than 84.9%in the control group with a statistically significant difference between the two group(P<0.05).Conclusion Fire needling plus medication has a definite effect on neurodermatitis.It can effectively reduce inflammatory reactions and the patients’anxiety and depression.
