老年心房颤动射频消融术后复发患者社会支持状况及其影响因素

摘　　要：目的观察心房颤动(Atrial fibrillation,AF)经射频消融术(Radio frequency catheter ablation,RFCA)治疗后复发老年患者的社会支持状况,并分析患者社会支持状况的影响因素。方法选择2017年1月至2019年12月本院收治的RFCA术后老年AF复发患者300例,采用本院自制调查表评估患者社会支持状况,单因素及多因素分析老年AF复发患者社会支持低水平的影响因素。结果RFCA术后老年AF复发患者的社会支持评定量表(SSR)评分为(36.72±15.86)分,整体社会支持水平处于中等水平;单因素分析结果显示,居住地、AF类型与RFCA术后老年AF复发患者社会支持低水平无关(P>0.05);年龄、性别、文化程度、婚姻状况、家庭经济状况、家属人数、家庭关系、医疗费用支付方式均可能是RFCA术后老年AF复发患者社会支持低水平的影响因素(P<0.05);多项Logistic回归分析结果显示,年龄≥70岁、男性、无配偶或离异、不能负担医疗费用、自费支付医疗费用均是RFCA术后老年AF复发患者社会支持低水平的危险因素(OR>1,P<0.05);文化程度大学及以上、家属人数≥3、家庭关系好是RFCA术后老年AF复发患者社会支持水平的保护因素(OR<1,P<0.05)。结论RFCA术后老年AF复发患者的整体社会支持水平处于中等水平,社会支持低水平受年龄、性别、配偶、文化程度、家庭经济状况、家属人数、家庭关系、医疗费用支付方式多种因素影响,应全面收集患者资料,对具有高危因素的患者针对性提供社会支持。Objective To observe the social support status of elderly patients with recurrence after atrial fibrillation(AF)treated by radio frequency catheter ablation(RFCA),and analyze the influencing factors of the patients'social support status.Methods A total of 300 elderly patients with relapsed AF after RFCA in our hospital from January 2017 to December 2019 were selected.The self-made questionnaire of our hospital was used to assess their social support status,single factors and multi factors analysis were used to analyze the influencing factors of low level of social support in elderly patients with AF relapse.Results The social support rating scale(SSR)score of elderly patients with AF recurrence after RFCA surgery was(36.72±15.86),and the overall social support level was at a medium level;preliminary single factor test results showed that residence,AF type and RFCA was irrelevant with the low level of social support for postoperative elderly AF relapse patients(P>0.05);age,gender,education,marital status,family economic status,number of family members,family relationships,and ways of paying medical expenses may all be risk factors of low level of social support in elderly patients with recurrent AF after RFCA(P<0.05);multiple logistic regression analysis results showed that age≥70 years old,male,without spouse or divorce,unable to afford medical expenses,and paying medical expenses at their own expense were all the risk factors of low level of social support in elderly patients with recurrent AF after RFCA(OR>1,P<0.05);education level of college or above,number of family members≥3,good family relationship were protective factors for the level of social support for elderly AF relapsed patients after RFCA(OR<1,P<0.05).Conclusion The overall social support level of elderly patients with AF recurrence after RFCA is at a medium level.The low level of social support is affected by many factors such as age,gender,spouse,education,family economic status,number of family members,family relations,and medical payment me
