摘 要:目的探讨男性2型糖尿病(type 2 diabetes mellitus,T2DM)患者骨密度随年龄变化趋势及相关影响因素。方法回顾性分析2018年1月至2019年12月在宁夏回族自治区人民医院住院并进行骨密度检查的64例男性T2DM患者的临床资料。50岁以前分为一个年龄段,50岁以后按每10岁分段,比较各年龄段骨密度(bone mineral density,BMD)的差异,同时分别按体质量指数(bone mass index,BMI)、血25羟维生素D(25 hydroxyvitamin D,25OHD)水平、胰岛素抵抗指数(HOMA-IR)分组,比较不同组间骨密度差异。结果单因素方差分析结果显示,各年龄组间骨密度差异无统计学意义(P>0.05)。各体质量指数组间腰椎及股骨颈骨密度差异无统计学意义(P>0.05),全髋骨密度差异有统计学意义(P<0.05)。25OHD缺乏组与非缺乏组比较,骨密度差异均无统计学意义(P>0.05)。胰岛素抵抗组与非胰岛素抵抗组比较,骨密度差异无统计学意义(P>0.05)。结论男性T2DM患者随着年龄的增长骨密度缓慢降低,不同年龄阶段骨密度差异不大。男性T2DM患者体质量指数、胰岛素抵抗、25OHD水平等与骨密度无确切相关关系。Objective To explore the relationship of bone mineral density(BMD)and age in male patients with type 2 diabetes mellitus(T2DM)and related influencing factors.Methods A retrospective analysis was conducted in 64 male patients with T2DM who were hospitalized in our hospital from January 2018 to December 2019 and underwent bone mineral examination.Patients younger than 50 years old was classified to a group.Patients over 50 years were divided into different groups for every 10 years old.BMD was compared among different groups.In addition,patients were also divided to different groups according to body mass index(BMI),serum 25 hydroxyvitamin D(25OHD)level,and insulin resistance index(HOMA-IR).BMD was compared between different groups.Results Univariate analysis of variance showed that there was no significant difference in BMD among different groups with different age(P>0.05).There was no statistically significant difference in BMD of spine and femoral neek among different BMI groups(P>0.05),but the total hip bone density difference was statistically significant(P<0.05).No significant difference was observed between groups divided by 25OHD and insulin resistance(both P>0.05).Conclusion Male T2DM patients show a slow decrease in BMD with age,but no significant difference is observed among different age groups.Body mass index,insulin resistance,and 25OHD level of male T2DM patients have no definite correlation to BMD.
