摘　　要：目的:建立本院静脉用药调配中心(PIVAS)全程化药学服务模式,进一步促进临床静脉用药安全。方法:建立系统的PIVAS药师规范化培训模式,提高药师专业技术水平;推行“药师+智慧软件”的PIVAS规范化全医嘱审核模式,提升药师审方效率;把握安全用药主线,确保精准高效的药品调配;加强临床调研,搭建“医-药-护-患”互动平台;开展用药教育及药学科普活动,不断扩大药学服务受众群体,形成药学服务闭环管理模式。结果:PIVAS药师提供的完整的闭环式药学服务,使患者用药安全得以保障,初步实现了基于用药安全的全程化药学服务模式。结论:全程化药学服务模式是药师工作和业务水平的充分体现,可进一步促进医院诊疗活动模式转变,进而提供优质、安全、人性化的药学服务。Objective:A new pharmacy intravenous admixture services(PIVAS)whole-process pharmaceutical care model was established in our hospital to promote the safety of clinical intravenous drug use.Methods:A systematic standardized training mode was built to improve the pharmacists professional technical level.A PIVAS prescription review mode,based on“pharmacist+intelligent software”,was carried out to improve prescription review efficiency.Grasp the main line of safe drug use to ensure accurate and efficient drug dispensing.Strengthen clinical research,and build a“doctor-pharmacist-nurse-patient”interactive platform.The drug education and pharmaceutical science popularization activities were provided to constantly expand the audience of pharmaceutical care,and form a closed-loop management mode of pharmaceutical care.Results:The complete closed-loop pharmaceutical care provided by PIVAS pharmacists to guarantee the drug safety of patients,and the whole pharmaceutical care mode based on drug safety can be initially realized.Conclusion:The whole-process pharmaceutical care model is the full embodiment of professional level of pharmacists.It can further promote the transformation of the mode of hospital diagnosis and treatment,and then provide quality,safe and humanized pharmaceutical care.
