高龄孕产妇风险评估及妊娠期保健的临床意义探讨

摘　　要：目的:探讨高龄孕产妇风险评估及妊娠期保健的临床意义。方法:选取2018年1月-2020年1月接受管理的高龄孕产妇154例,根据是否全程参与妊娠期保健分为高龄孕产妇保健组122例和高龄孕产妇未保健组32例;同时选取同期全程参与妊娠期保健适龄孕产妇100例,作为适龄保健组。比较三组孕产妇妊娠结局、并发症及新生儿情况。结果:高龄孕产妇未保健组早产率、过期产率、剖宫产率、产后出血率及妊娠期并发症发生率均明显高于高龄孕产妇保健组和适龄保健组,差异有统计学意义(P<0.05);高龄孕产妇未保健组死胎率、新生儿窒息率、巨大儿发生率和低体重儿发生率均明显高于高龄孕产妇保健组和适龄保健组,差异有统计学意义(P<0.05)。结论:高龄孕产妇风险评估及妊娠期保健对降低孕产妇剖宫产率、早产、过期产及巨大儿、低体重儿发生率及减少并发症有重要临床意义。Objective:To explore the clinical significance of risk assessment for advanced-age pregnant women and health care during pregnancy.Methods:From January 2018 to January 2020,154 advanced-age pregnant women who underwent management were selected.According to whether they participated in the whole process of pregnancy care,they were divided into the advanced-age pregnant women witho health care group with 122 cases and the advanced-age pregnant women without health care group with 32 cases.100 cases of age-appropriate pregnant women participated in the whole process of health care during pregnancy as the age-appropriate health care group.The pregnancy outcomes,comorbidities and neonatal conditions of the three groups were compared.Results:The premature delivery rate,post-term delivery rate,cesarean section rate,postpartum hemorrhage rate and pregnancy comorbidity rate in the advanced-age pregnant women without health care group were significantly higher than those in the advanced-age pregnant women with health care group and the age-appropriate health care group,the difference was statistically significant(P<0.05).The stillbirth rate,neonatal asphyxia rate,macrosomia rate,and low birth weight infant rate in the advanced-age pregnant women without health care group were significantly higher than those in the advanced-age pregnant women with health care group and the age-appropriate health care group,the difference was statistically significant(P<0.05).Conclusion:The risk assessment and pregnancy health care of the advanced-age pregnant women have important clinical significance in reducing the rate of cesarean section,reducing comorbidities,reducing premature delivery and post-term delivery,and reducing the incidence of large and low birth weight infants.
