摘　　要：目的:分析自身免疫性溶血性贫血(AIHA)患者中多种红细胞抗原相合性输血的应用效果。方法:2015年1月-2018年12月收治AIHA患者98例,所有患者放散液及血清均接受同种或同种抗体的鉴定,并且红细胞通过抗人球蛋白实验进行检测;根据输注方式不同分为两组。对照组采用ABO、Rh血型系统抗原相结合的主侧配血供者血液,观察组则给予ABO、Rh、Kidd、Duff、MNSs、lewis血型系统抗原结合的主侧配血供者血液。比较两组输血效果。结果:观察组输血后血细胞比容(HCT)及输血前后血红蛋白(Hb)水平均明显高于对照组,差异有统计学意义(P<0.05);观察组输血总有效率明显高于对照组,差异有统计学意义(P<0.05);并且观察组无不良反应事件出现。结论:针对AIHA患者在输血治疗中采用多种红细胞抗原相合性输血方法,能够显著提高输血安全性及有效性,改善患者的免疫反应及状态,明显减少抗体的产生。Objective:To analyze the application effect of multiple red blood cell antigen compatible blood transfusion in patients with autoimmune hemolytic anemia(AIHA).Methods:From January 2015 to December 2018,98 cases of AIHA patients were enrolled,all patients'fluids and sera were identified as the same or the same antibody,and the red blood cells were tested by anti-human globulin experiments.According to different infusion methods,the patients were divided into two groups.The control group was given the blood of the primary matching donor combined with ABO and Rh blood group system antigens,and the observation group was given the blood of the primary matching donor combined with ABO,Rh,Kidd,Duff,MNSs,and lewis blood group system antigens.The transfusion effects between two groups was compared.Results:The hematocrit(HCT)and hemoglobin(Hb)levels before and after blood transfusion in the observation group were significantly higher than those in the control group,the differences were statistically significant(P<0.05).The total efficiency rate of blood transfusion of the observation group was significantly higher than in the control group,the difference was statistically significant(P<0.05).No adverse reaction events occurred in the observation group.Conclusion:For AIHA patients,the use of multiple red blood cell antigen-compatible blood transfusion methods for blood transfusion treatment can significantly improve the safety and effectiveness of blood transfusion,greatly improve the patient's immune response and status,and significantly reduce antibody production.It has clinical application value.
