H型高血压患者颈动脉内中膜厚度与Hcy、Cyst-C、UA、Lp(a)的关系探讨

摘　　要：目的探究H型高血压患者颈动脉内中膜厚度与同型半胱氨酸(Hcy)、胱抑素-C(Cyst-C)、尿酸(UA)、脂蛋白(a)[Lp(a)]的关系。方法100例H型高血压患者,均进行二维超声检查,依照我国颈动脉粥样硬化标准分为A组[颈动脉正常(以颈动脉内中膜厚度<1.0 mm为正常),40例]、B组[颈动脉内中膜增厚(以颈动脉内中膜厚度1.0~1.5 mm为增厚),30例]、C组[颈动脉内斑块形成(以颈动脉内中膜厚度>1.5 mm或局部隆起超过正常组织50%或凸入管腔≥2.5 mm为斑块形成),30例]。三组患者均进行血清生化指标检测,对比三组患者Hcy、Cyst-C、UA、Lp(a)水平。结果A组患者Hcy、Cyst-C、UA、Lp(a)水平分别为(14.57±3.21)μmol/L、(1.04±0.40)mg/L、(351.77±43.15)μmol/L、(250.38±40.39)mg/L,B组Hcy、Cyst-C、UA、Lp(a)水平分别为(19.25±3.45)μmol/L、(1.39±0.39)mg/L、(389.78±44.64)μmol/L、(320.24±42.37)mg/L,C组患者Hcy、Cyst-C、UA、Lp(a)水平分别为(23.75±4.41)μmol/L、(1.62±0.23)mg/L、(425.91±62.13)μmol/L、(390.58±50.38)mg/L;B组、C组患者Hcy、Cyst-C、UA、Lp(a)水平均高于A组,C组患者Hcy、Cyst-C、UA、Lp(a)水平均高于B组,差异有统计学意义(P<0.05)。结论H型高血压患者Hcy、Cyst-C、UA、Lp(a)水平可随颈动脉内中膜增厚而升高,检测此类指标可有效反映患者颈动脉粥样硬化程度,为临床诊治提供科学依据,并预防H型高血压患者心脑血管疾病发生。Objective To investigate the correlation between carotid intima-media thickness and homocysteine(Hcy),cystatin-C(Cyst-C),uric acid(UA),lipoprotein(a)[Lp(a)]in patients with H-type hypertension.Methods A total of 100 patients with H-type hypertension were examined by two-dimensional ultrasound,and were divided into group A[normal carotid artery(carotid intima-media thickness<1.0 mm),40 cases],group B[thickening carotid intima-media(carotid intima-media thickness 1.0-1.5 mm),30 cases],group C[carotid artery plaque formation(carotid intima-media thickness≥1.5 mm or local bulge exceeding 50% of normal tissue or protruding into the lumen≥2.5 mm),30 cases]according to the national carotid atherosclerosis standard.The serum biochemical indicators of the three groups were detected,and the levels of Hcy,Cyst-C,UA,and Lp(a)in the three groups were compared.Results The levels of Hcy,Cyst-C,UA and Lp(a)of group A were(14.57±3.21)μmol/L,(1.04±0.40)mg/L,(351.77±43.15)μmol/L and(250.38±40.39)mg/L,which were(19.25±3.45)μmol/L,(1.39±0.39)mg/L,(389.78±44.64)μmol/L and(320.24±42.37)mg/L of group B,and(23.75±4.41)μmol/L,(1.62±0.23)mg/L,(425.91±62.13)μmol/L and(390.58±50.38)mg/L of group C.The levels of Hcy,Cyst-C,UA and Lp(a)of group B and group C were higher than those of group A,and group C was higher than group B.All the difference was statistically significant(P<0.05).Conclusion The levels of Hcy,Cyst-C,UA,and Lp(a)in patients with H-type hypertension can increase with the thickening of the carotid intima-media.The detection of such indicators can effectively reflect the degree of carotid atherosclerosis in patients,provide scientific basis for clinical diagnosis and treatment,and prevent the occurrence of cardiovascular and cerebrovascular diseases in patients with H-type hypertension.
