摘 要:目的分析多层螺旋CT(MSCT)扫描与CT血管造影技术(CTA)对肾脏以及肾血管疾病的诊断价值。方法78例肾病或肾血管病变患者作为研究对象,均接受MSCT扫描以及CTA技术扫描,做好数据记录,选取两名临床经验丰富的放射科医师对所得图像开展共同评估,分析MSCT扫描图像与CTA重建图像在肾脏以及肾血管疾病中的检查优势。结果在MSCT扫描的78份图像中,共检出48例病灶结节,其中30例为肾实质病变(62.50%)、8例为囊性病变(16.67%)、9例为肾畸形(18.75%)、1例为肾动脉血栓(2.08%),检出肾血管血栓个数较低。CTA技术扫描检出60例病灶结节,其中30例为肾实质病变(50.00%)、8例为囊性病变(13.33%)、9例为肾畸形(15.00%)、13例为肾血管病变(21.67%)。CTA技术检出病灶率明显高于MSCT,差异具有统计学意义(P<0.05)。MSCT肾实质病变检出率较高,但在肾血管病变中检出效果一般。结论MSCT扫描以及CTA技术均可直观显示肾实质结构变化,其中CTA技术能够直观、全面、准确地显示肾实质以及肾血管病变区域,沿多角度展现肾脏与其血管之间的空间解剖关系,有助于影像学医师精准定位病变部位,具体掌握患者肾血管解剖位置,指导医生制定正确的治疗方案,值得在临床上推广应用。Objective To analyze the diagnostic value of multi-slice spiral CT(MSCT)scan and CT angiography(CTA)in nephropathy and renal vascular diseases.Methods 78 patients with nephropathy or renal vascular disease were selected as the research subjects.They were all scanned by MSCT and CTA technology,and the data were recorded.2 experienced radiologists were selected to evaluate the obtained images,and the advantages of MSCT scanning images and CTA reconstruction images in the examination of nephropathy and renal vascular diseases were analyzed.Results In 78 images scanned by MSCT,48 cases of nodules were detected,of which 30 cases(62.50%)were renal parenchymal lesions,8 cases(16.67%)were cystic lesions,and 9 cases(18.75%)were renal malformations,1 case(2.08%)was renal artery thrombosis,and the number of renal blood clots detected was low.CTA scanning detected 60 cases of nodules,of which 30 cases were renal parenchymal lesions(50.00%),8 cases(13.33%)were cystic lesions,9 cases(15.00%)were renal malformations,and 13 cases(21.67%)were renal vascular lesions.The number of lesions detected by CTA was obviously higher than that of MSCT,and the difference was statistically significant(P<0.05).MSCT had a high detection rate of renal parenchymal lesions,but the detection effect in renal vascular lesions was not very good.Conclusion Both MSCT scan and CTA can fully show the changes of renal parenchyma structure.The CTA technology can directly,comprehensively and accurately show the renal parenchyma and the area of renal vascular lesions,and show the spatial anatomical relationship between the kidney and its vessels from multiple angles,which is conducive to the accurate positioning of the lesion site by the imaging physicians,and grasp the clear anatomical shape of the renal vessels of the patients,and guide the doctor to formulate the correct treatment plan.It is worthy of clinical promotion and application.
