参附注射液应用于外伤性脾破裂患者腹腔镜脾切除术后的效果分析

摘　　要：目的探讨参附注射液应用于外伤性脾破裂患者腹腔镜脾切除术后的效果。方法78例外伤性脾破裂进行腹腔镜脾切除术的患者,按照随机数字法将其分为对照组和观察组,每组39例。对照组实施常规术后治疗,观察组在对照组的基础上给予参附注射液治疗。比较两组患者手术前后的血清胃泌素水平,术后不同时间的肠鸣音次数,术后肛门排气时间、排便时间。结果观察组患者术后1、2、3 d的血清胃泌素水平分别为(350.11±41.25)、(330.57±41.23)、(374.83±60.19)ng/L,高于对照组的(310.18±43.62)、(258.03±45.75)、(329.31±54.46)ng/L,差异具有统计学意义(P<0.05)。观察组患者术后6、12、24、48 h的肠鸣音次数多于对照组,差异具有统计学意义(P<0.05)。观察组患者的术后肛门排气时间(31.19±5.33)h、术后排便时间(42.15±8.31)h短于对照组的(45.22±7.71)、(55.43±11.51)h,差异具有统计学意义(P<0.05)。结论外伤性脾破裂患者腹腔镜脾切除术后加用参附注射液可优化血清胃泌素水平,增加肠鸣音次数,缩短肛门排气、排便时间,术后肠道功能可快速恢复。Objective To discuss the effect of Shenfu Injection on patients with traumatic splenic rupture after laparoscopic splenectomy.Methods A total of 78 patients with traumatic splenic rupture after laparoscopic splenectomy were divided into control group and observation group according to random numerical table,with 39 cased in each group.The control group received routine postoperative treatment,and the observation group received Shenfu Injection on the basis of the control group.The serum gastrin levels before and after surgery,frequency of bowel sounds at different time after surgery,anal exhaust time after surgery and defecation time were compared between the two groups.Results At 1,2 and 3 d after surgery,the serum gastrin levels of the observation group were(350.11±41.25),(330.57±41.23)and(374.83±60.19)ng/L,which were higher than(310.18±43.62),(258.03±45.75)and(329.31±54.46)ng/L of the control group,and the difference was statistically significant(P<0.05).At 6,12,24 and 48 h after surgery,the frequency of bowel sounds of the observation group were more than those of the control group,and the difference was statistically significant(P<0.05).The exhaust time after surgery(31.19±5.33)h and defecation time after surgery(42.15±8.31)h of the observation group were shorter than(45.22±7.71)and(55.43±11.51)h of the control group,and the difference was statistically significant(P<0.05).Conclusion Shenfu injection can optimize the level of serum gastrin,increase the frequency of bowel sounds,shorten the time of anal exhaust and defecation,and quickly recover the intestinal function of patients with traumatic splenic rupture after laparoscopic splenectomy.
