以急性心肌梗死为主要表现的不典型主动脉夹层1例报告

摘　　要：1例51岁男性患者以急性心肌梗死为主要表现入院,行经皮冠状动脉介入术(PCI)治疗后患者反复出现心力衰竭。影像检查结果均提示主动脉增宽、心包积液,主动脉反流呈进行性加重。主动脉瓣置换术中发现患者右冠窦及无冠窦夹层形成,改行带主动脉瓣人工血管升主动脉替换术(Bentall术)及右冠状动脉搭桥术,术后患者心功能恢复良好,无明显不适。A 51-year-old male patient was admitted to hospital due to main symptom of acute myocardial infarction,and occurred repeated heart failure after percutaneous coronary intervention(PCI),and the imaging examination results showed aortic widening and pericardial effusion.Aortic regurgitation was progressively aggravated,and the right coronary sinus and no coronary sinus dissection were found in the patient during the aortic valve replacement,so the patient underwent ascending aortic replacement with artificial aortic valve(Bentall procedure)and right coronary artery bypass grafting with aortic valve artificial vessel.Cardiac function of the patient recovered well after the surgery without obvious discomfort.
