摘 要:目的:研究比较头颈肩塑形垫+头颈肩面罩、真空垫+头颈肩面罩、热缩枕+头颈肩面罩、单独头颈肩面罩4种固定技术在头颈部肿瘤放疗中的摆位误差。方法:选取2018年4月至2020年4月来某院放疗的头颈部肿瘤患者120例,根据固定方式分为4组,每组30人。A组采用头颈肩塑形垫+头颈肩面罩固定方式,B组采用真空垫+头颈肩面罩固定方式,C组采用热缩枕+头颈肩面罩固定方式,D组采用单独头颈肩面罩固定方式。统计各组左右X方向(Lat)、头脚Y方向(Lng)、腹背Z方向(Vrt)、旋转方向(Rtn)的摆位误差数值,分析4种不同体位固定方式的差异。采用SPSS 22.0软件进行统计学分析。结果:A组与B组在Lat、Lng、Vrt方向摆位误差的平均值小于1 mm,在Rtn方向摆位误差的平均值小于0.2°;C组与D组在Lat、Lng、Vrt方向摆位误差的平均值大于1 mm,在Rtn方向摆位误差的平均值大于0.2°,但均满足固定要求。A组在各方向的摆位误差小于C、D组,在Lat、Lng、Vrt方向的摆位误差小于B组(P均<0.5),但A组与B组在Rtn方向的摆位误差比较差异无统计学意义(P>0.05)。B组在各方向的摆位误差小于D组,在Vrt、Rtn方向的摆位误差小于C组(P均<0.05),B组与C组在Lat、Lng方向的摆位误差比较差异无统计学意义(P>0.05)。C组在Lng、Vrt方向的摆位误差小于D组(P<0.05),但C组与D组在Lat、Rtn方向的摆位误差比较差异无统计学意义(P>0.05)。结论:4种固定技术中,头颈肩塑形垫+头颈肩面罩固定的重复性和稳定性最好,值得临床推广应用。Objective To explore the setup errors during the radiotherapy of head and neck tumors by the head,neck and shoulder mask and those by the mask combined with the plastic cushion,vacuum cushion or heat-shrinkage pillow.Methods Totally 120 head and neck tumor patients undergoing radiotherapy in some hospital from April 2018 to April 2020 were selected and divided into four groups of 30 patients each according to the immobilization technique,that is,group A,B,C and D.The head,neck and shoulder mask was used in combination with the plastic cushion in group A for immobilization,with the vacuum cushion in group B,with the heat-shrinkage pillow in group C and with no other means in group D.The values of the setup errors in X(Lat),Y(Lng),Z(Vrt)and rotation(Rtn)directions were counted for each group to analyze the differences between the four different immobilization techniques.SPSS 22.0 software was used for statistical analysis.Results The mean values of the setup errors in Lat,Lng and Vrt directions of group A and group B were lower than 1 mm,and the errors in Rtn direction were less than 0.2°;the mean values of the setup errors in Lat,Lng and Vrt directions of group C and group D were higher than 1 mm,and the errors in Rtn direction were higher than 0.2°,which all met the requirements for immobilization.Group A had the setup errors in all the directions lower than those of group C and D,and also less than those of group B in Lat,Lng and Vrt directions(all P<0.05),while in Rtn direction group A and B had no statistically different setup errors(P>0.05).Group B had the setup errors in all the directions lower than those of group D,and also less than those of group C in Vrt and Rtn directions(all P<0.05),while in Lat and Lng directions group B and C had no statistically different setup errors(P>0.05).Group C had the setup errors in Lng and Vrt directions lower than those of group D(all P<0.05),while in Lat and Rtn directions group C and D had no statistically different setup errors(P>0.05).Conclusion The plastic cushion
