摘  要:目的分析重症急性胰腺炎(SAP)外周血微小核糖核酸-9(miR-9)、miR-155的表达水平及与血清淀粉酶(S-amy)、炎性反应、辅助性T细胞17(Th17)/调节性T细胞(Treg)平衡的关系。方法选择2015年3月—2020年5月中国人民解放军西藏军区总医院急诊医学科收治AP患者294例(病例组)及同期在医院进行体检的健康者51例(健康对照组)为研究对象。其中重度AP患者39例为SAP亚组,中度AP患者97例为MSAP亚组,轻度AP患者158例为MAP亚组。采用实时荧光定量聚合酶链式反应(RT-qPCR)技术检测2组外周血miR-9和miR-155表达量;流式细胞仪检测Treg、Th17水平,计算Th17/Treg比值;酶联免疫吸附试验(ELISA)测定白介素6(IL-6)、IL-8、肿瘤坏死因子α(TNF-α)、C反应蛋白(CRP)水平;酶速率法检测血清S-amy水平。分析miR-9、miR-155与IL-6、IL-8、TNF-α、CRP、Th17细胞、Treg细胞、Th17/Treg、S-amy的关系。结果miR-9、miR-155、IL-6、IL-8、TNF-α、CRP、S-amy、Th17细胞、Th17/Treg水平比较,SAP亚组>MSAP亚组>MAP亚组>健康对照组,差异均有统计学意义(F/P=19.574/0.000、16.832/0.000、63.472/0.000、87.546/0.000、45.871/0.000、21.873/0.000、77.965/0.000、19.375/0.000、14.682/0.000)。Treg细胞水平比较,SAP亚组<MSAP亚组<MAP亚组<健康对照组,差异有统计学意义(F/P=25.473/0.000)。miR-9、miR-155表达与IL-6、IL-8、TNF-α、CRP、Th17细胞、Th17/Treg、S-amy水平呈正相关(miR-9:r/P=0.527/0.024、0.543/0.022、0.535/0.023、0.716/0.002、0.624/0.009、0.754/0.000、0.518/0.027;miR-155:r/P=0.525/0.025、0.517/0.029、0.524/0.026、0.685/0.002、0.632/0.008、0.734/0.001、0.522/0.025),与Treg细胞呈负相关(r/P=-0.633/0.007、-0.632/0.009)。结论SAP患者外周血miR-9、miR-155的相对表达量显著上调,其与机体炎性反应和Th17/Treg失衡关系密切,可能参与了SAP的病理改变过程。Objective To analyze the expression levels of microribonucleic acid 9(miR-9)and miR155 in peripheral blood of severe acute pancreatitis(SAP)and their relationship with serum amylase(S-amy),inflammatory response,helper T cell 17(Th17)/regulatory T cell(Treg)Balanced relationship.Methods From March 2015 to May 2020,294 AP patients admitted to the Emergency Medicine Department of the General Hospital of Tibet Military Command of the Chinese People's Liberation Army(case group)and 51 healthy patients who underwent physical examinations in the hospital during the same period(healthy control group)were selected as the research objects.Among them,39 patients with severe AP were in the SAP subgroup,97 patients with moderate AP were in the MSAP subgroup,and 158 patients with mild AP were in the MAP subgroup.Real-time fluorescent quantitative polymerase chain reaction(RT-qPCR)technology was used to detect the expression of miR-9 and miR-155 in peripheral blood of the two groups.Flow cytometry detects the levels of Treg and Th17,and calculates the ratio of Th17/Treg.Enzyme-linked immunosorbent assay(ELISA)measures the levels of interleukin 6(IL-6),IL8,tumor necrosis factorα(TNF-α),and C-reactive protein(CRP).The enzyme rate method was used to detect the serum Samy level.Analyze the relationship between miR-9,miR-155 and IL-6,IL-8,TNF-α,CRP,Th17 cells,Treg cells,Th17/Treg,and S-amy.Results Comparing the levels of miR-9,miR-155,IL-6,IL-8,TNF-α,CRP,S-amy,Th17 cells,Th17/Treg,SAP subgroup>MSAP subgroup>MAP subgroup>healthy control group,the difference was statistically significant(F/P=19.574/0.000,16.832/0.000,63.472/0.000,87.546/0.000,45.871/0.000,21.873/0.000,77.965/0.000,19.375/0.000,14.682/0.000).Comparison of Treg cell levels,SAP subgroup<MSAP subgroup<MAP subgroup<healthy control group,the difference was statistically significant(F/P=25.473/0.000).The expression of miR9 and miR155 was positively correlated with the levels of IL-6,IL-8,TNF-α,CRP,Th17 cells,Th17/Treg,and S-amy(miR-9:r/P=0.527/0.024,0.543/0.022,0.535/0.
