摘 要:目的分析突发性耳聋患者脑干听觉诱发电位(BAEP)的改变。方法收集2017年1月至2020年12月期间在菏泽市第三人民医院治疗的突发性耳聋患者62例,采用诱发电位肌电图仪检测BAEP,分析患者(患侧和健侧)与对照组的潜伏期(PL)、峰间期(IPL)、波幅(Amp)等参数。结果患者患侧各波PL较健侧和对照组显著延长(P<0.05);患者健侧波ⅤPL较对照组显著延长(P<0.05)。患侧Ⅲ-Ⅴ、Ⅰ-Ⅴ波IPL较健侧和对照组(P<0.05)、健侧较对照组(P<0.05)均显著延长。患侧波Ⅰ、波Ⅴ的Amp与健侧比较差异有统计学意义(P<0.05),而Ⅴ/ⅠAmp比值患侧则显著地高于健侧(P<0.05)。各波潜伏期两侧差(ILD)≥0.4 ms出现率在32.61%~40.38%,在各峰间期两侧差(IILD)≥0.4 ms的病例中,有部分病例IILD出现了负值。单侧耳聋健侧的异常主要为IPLⅠ-Ⅴ波延长、波ⅤAmp降低。结论BAEP检测是一项客观敏感的神经电生理学方法,对于评价突发性耳聋的听觉传导功能状况具有重要价值。Objective To analyze the changes of brainstem auditory evoked potential(BAEP)of patients with sudden deafness.Methods From January 2017 to December 2020,62 patients with sudden deafness were collected.The BAEPs of the subjects were detected by Nicolet Viking quest evoked potential electromyography,and the parameters such as peak latency(PL),interpeak latency(IPL)and amplitude(AMP)of the patients were analyzed by SPSS24.0 software.Results The PL of the deaf side of the patients was significantly longer than that of the healthy side of the patients and the control group(P<0.05).The VPL of the healthy side of the patients was significantly longer than that of the control group(P<0.05).WavesⅢ-ⅤandⅠ-Ⅴof the IPL of the deaf side of the patients were significantly longer than those of the healthy side and the control group(P<0.05),WavesⅢ-ⅤandⅠ-Ⅴof the healthy side was significantly longer than those of the control group(P<0.05).The difference of Wave I and Wave V in AMP between the deaf side and the healthy side was statistically significant(P=0.000-0.001),while the ratio of V/I AMP in the deaf side was significantly higher than that in the healthy side(P<0.05).The occurrence rate of wave interlateral difference(IDL)≥0.4ms was 30.36%~40.38%,and the negative value was found in some of the cases with interlateral interpeak latensy difference(IILD)≥0.4ms.The main abnormalities in the healthy side of the cases with unilateral deafness included the prolongation of IPLⅠ-Ⅴand the decrease of WaveⅤAMP.Conclusion BAEP is an objective and sensitive neuroelectrophysiological method and of great value in the evaluation of the auditory conduction function of sudden deafness.
