磷酸川芎嗪联合尼莫地平治疗老年颅内动脉瘤夹闭术后早期脑缺血患者的效果

摘　　要：目的:观察磷酸川芎嗪联合尼莫地平治疗老年颅内动脉瘤夹闭术后早期脑缺血患者的效果。方法:选取90例老年颅内动脉瘤夹闭术后早期脑缺血患者为研究对象,按照随机数字表法分为对照组和观察组,各45例。对照组给予尼莫地平治疗,观察组在对照组基础上联合磷酸川芎嗪治疗。比较两组临床疗效,治疗前后格拉斯哥昏迷量表(GCS)评分、美国国立卫生研究院卒中量表(NIHSS)评分、脑动脉[大脑中动脉(MCA)、大脑前动脉(ACA)、大脑后动脉(PCA)、基底动脉(BA)]血流速度指标水平和不良反应发生率。结果:观察组治疗总有效率82.22%,高于对照组的62.22%,差异有统计学意义(P<0.05);治疗后,观察组GCS评分高于对照组,NIHSS评分低于对照组,差异均有统计学意义(P<0.05);观察组MCA、ACA、PCA、BA血流速度低于对照组,差异均有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:磷酸川芎嗪联合尼莫地平治疗老年颅内动脉瘤夹闭术后早期脑缺血患者可提高治疗总有效率,改善GCS评分、NIHSS评分和脑动脉血流速度,效果优于单纯应用尼莫地平治疗。Objective:To investigate effects of Ligustrazine phosphate combined with Nimodipine in treatment of elderly patients with early postoperative cerebral ischemia after clipping of intracranial aneurysm.Methods:90 elderly patients with early postoperative cerebral ischemia after clipping of intracranial aneurysm were selected as the research objects,and were divided into control group and observation group according to the random number table method,45 cases in each group.The control group was treated with Nimodipine,while the observation group was treated with Ligustrazine phosphate on the basis of that of the control group.The clinical efficacy,the scores of Glasgow coma scale(GCS)and national institutes of health stroke scale(NIHSS)score,the cerebral artery blood flow velocity index[middle cerebral artery(MCA),anterior cerebral artery(ACA),posterior cerebral artery(PCA),basilar artery(BA)]levels before and after the treatment and incidence of adverse reactions were compared between the two groups.Results:The effective rate of treatment in the observation group was 82.22%,which was higher than 62.22%in the control group,and the difference was statistically significant(P<0.05).After the treatment,the GCS score of the observation group was higher than that of the control group;the NIHSS score was lower than that of the control group;and the differences were statistically significant(P<0.05).The blood flow velocities of MCA,ACA,PCA and BA in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Ligustrazine phosphate combined with Nimodipine in the treatment of the elderly patients with early postoperative cerebral ischemia after clipping of intracranial aneurysm can increase the total effective rate of treatment and improve the GCS score,the NIHSS score and the cerebral arterial blood flow velocity.Moreover,it
