轻比重腰麻与神经阻滞麻醉在老年髋关节手术中的应用

摘　　要：目的探讨老年髋关节手术患者分别实施轻比重腰麻以及神经阻滞麻醉的临床效果。方法方便选取该院2017年6月—2020年5月收治的58例老年髋关节手术患者,按数字奇偶法分组;腰麻组(29例):采用轻比重腰麻方法完成髋关节手术麻醉;神经组(29例):采用神经阻滞麻醉方法完成髋关节手术麻醉。就组间平均动脉压、心率、感觉阻滞持续时间、麻醉起效时间、运动阻滞持续时间展开对比。结果麻醉前,腰麻组平均动脉压(96.25±15.25)mmHg、心率(71.99±15.25)次/min同神经组比较,差异无统计学意义(t=0.114、0.034,P>0.05);麻醉后5 min,腰麻组平均动脉压(95.11±15.13)mmHg、心率(71.89±15.13)次/min同神经组比较差异无统计学意义(t=0.338、0.561,P>0.05);麻醉后15 min,腰麻组平均动脉压(86.99±15.29)mmHg、心率(74.13±12.25)次/min同神经组比较差异无统计学意义(t=0.788、0.044,P>0.05);手术后,腰麻组平均动脉压(95.59±13.59)mmHg、心率(78.22±9.99)次/min同神经组比较差异无统计学意义(t=0.192、0.911,P>0.05);腰麻组感觉阻滞持续时间(211.59±32.69)min、麻醉起效时间(2.85±0.69)min、运动阻滞持续时间(72.25±17.13)min均短于神经组(450.13±86.25)min、麻醉起效时间(17.01±2.39)min、运动阻滞持续时间(303.55±75.13)min明显,差异有统计学意义(t=13.926、30.653、16.164,P<0.05)。结论轻比重腰麻方法有效运用,同神经阻滞麻醉比较,能够将患者感觉阻滞持续时间、麻醉起效时间、运动阻滞持续时间显著缩短,并且不会对患者心率以及平均动脉压造成严重影响,最终可促进老年髋关节手术患者手术成功以及有效预后。Objective To explore the clinical effects of light specific spinal anesthesia and nerve block anesthesia for elderly patients undergoing hip surgery.Methods Convenient selected 58 elderly patients undergoing hip surgery in the hospital from June 2017 to May 2020 were divided into groups by digital parity;spinal anesthesia group(29 cases):light specific gravity spinal anesthesia was used to complete hip surgery anesthesia;nerve group(29 cases):nerve block anesthesia was used to complete hip surgery anesthesia.The average arterial pressure,heart rate,duration of sensory block,onset time of anesthesia,and duration of motor block between groups were compared.Results Before anesthesia,the mean arterial pressure(96.25±15.25)mmHg and heart rate(71.99±15.25)beats/min of the spinal anesthesia group were not significantly different from those in the nerve group(t=0.114,0.034,P>0.05);5 min after anesthesia,the mean arterial pressure(95.11±15.13)mmHg and heart rate(71.89±15.13)beats/min of the spinal anesthesia group were not significantly different from those in the nerve group(t=0.338,0.561,P>0.05);15 min after anesthesia,the mean arterial pressure(86.99±15.29)mmHg and heart rate(74.13±12.25)beats/min of the spinal anesthesia group were not significantly different from those in the nerve group(t=0.788,0.044,P>0.05);after surgery,the mean arterial pressure(95.59±13.59)mmHg and heart rate(78.22±9.99)beats/min of the spinal anesthesia group were not significantly different from those in the nerve group(t=0.192,0.911,P>0.05);the duration of sensory block in the spinal anesthesia group(211.59±32.69)min,the onset time of anesthesia(2.85±0.69)min,and the duration of motor block(72.25±17.13)min were shorter than those in the nerve group(450.13±86.25)min,anesthesia onset time(17.01±2.39)min,and motor block duration(303.55±75.13)min,and the difference was statistically significant(t=13.926,30.653,16.164,P<0.05).Conclusion The effective use of light specific gravity spinal anesthesia,compared with nerve block anesthesia
