摘　　要：目的探讨经皮扩张气管切开术(PDT)和传统气管切开术(TT)用于不同体质量指数(BMI)患者的临床效果。方法前瞻性纳入2018-02—2020-03成都市4家三级综合医院重症医学科行气管切开术的患者,根据手术方法分为PDT组和TT组。比较2组患者的基线资料、手术时间、术中出血量,以及术后并发症发生率。结果共纳入146例患者,PDT组75例,TT组71例。2组患者的基线资料差异无统计学意义(P>0.05)。PDT组手术时间、术中出血量明显少于TT组,术后并发症发生率低于TT组,差异均有统计学意义(P<0.05)。亚组分析结果显示,随着BMI增加,TT组并发症发生率高于PDT组,差异有统计学意义(P<0.05)。结论PDT较TT迅速、安全有效、创伤小且并发症少,适用于不同BMI的患者。Objective To explore the clinical effect of percutaneous dilatation tracheotomy(PDT)and traditional tracheotomy(TT)for different body mass index(BMI)patients.Methods Prospectively included from 2018-02 to 2020-03 patients undergoing tracheotomy in 4 tertiary general hospitals in Chengdu were divided into PDT and TT groups according to the surgical methods.Baseline data,operative time,intraoperative blood loss and postoperative complications were compared between the two groups.Results A total of 146 patients,75 cases in PDT group,71 cases in TT group.Baseline data of 2 groups were not statistically significant(P>0.05).PDT group had less operation time and intraoperative bleeding than that of TT group,and the incidence of postoperative complications was lower than that of TT group,All the differences were statistically significant(P<0.05).Subgroup analysis showed,As BMI increases,The incidence of complications in TT group was higher than that of PDT group,Differences were statistically significant(P<0.05).Conclusion PDT comparing with TT is quickly,safe and effective,less trauma and less complications,suitable for patients with different BMI indices.
