摘 要:目的评价血清AQP5诊断ACO的临床价值。方法选取2017年3月至2020年7月在大连大学附属中山医院就诊的ACO患者30例、哮喘患者40例、COPD患者50例及未吸烟的健康者30例。ELISA检测受试者血清AQP5表达水平。单因素方差分析统计AQP5在对照组、哮喘组、COPD组、ACO组中表达水平与各因素之间的相关性。ROC曲线评价AQP5对ACOS的诊断价值。结果各组间年龄、肺功能存在统计学差异(P<0.05)。ACO组肺功能FEV_(1)、FVC、FEV_(1)/FVC值低于对照组、哮喘组、COPD组,但与COPD组无统计学差异(P>0.05);ACO组支气管舒张试验前后ΔFEV 1低于哮喘组,高于对照组和COPD组,两两比较有统计学差异(P<0.05)。ACO组AQP5表达水平(50.80±21.95)ng/mL,COPD组(81.44±17.69)ng/mL、哮喘组(107.00±17.33)ng/mL和对照组(183.8.±13.97)ng/mL,两两比较有统计学差异(P<0.001)。AQP5表达水平与肺功能正相关(P<0.05),与年龄和吸烟指数负相关(P<0.05)。回归分析显示吸烟指数、FEV_(1)、ΔFEV_(1)对AQP5表达有影响(P=0.001,P=0.000,P=0.001)。血清AQP5水平AUC为0.774(P<0.001)。结论AQP5对鉴别哮喘、COPD和ACO有重要意义。Objective To evaluate the clinical value of serum AQP5 in the diagnosis of ACO.Methods 30 patients with ACO,40 with asthma,50 with COPD and 30 non-smokers were selected from Zhongshan Hospital Affiliated to Dalian University from February 2017 to June 2020.The serum AQP5 level was detected by ELISA.Correlation between serum AQP5 expression levels and various factors was analyzed among the control group,the asthma group,the COPD group and the ACO group by Pearson lineage correlation analysis.The diagnostic value of serum AQP5 in ACO was evaluated by ROC.Results There were statistical differences in gender and lung function among the ACO group,the COPD group,the asthma group and the control group(P<0.05).The levels of FEV_(1),FVC and FEV_(1)/FVC were lower in the ACO group than those in the control group,the asthma group and the COPD group,but there was no significant difference between the ACO group and the COPD group(P>0.05).The changes ofΔFEV 1 in the ACO group was lower than that in the asthma group,but higher than that in the control group and the COPD group,and there were significant differences between any other two groups(P<0.05).The expression of AQP5 was(50.80±21.95 ng/mL)in the ACO group,(81.44±17.69ng/mL)in the COPD group,(107.00±17.33ng/mL)in the asthma group and(183.8.±13.97ng/mL)the control group,with statistical difference between any two groups(P<0.001).The expression of AQP5 in serum was positively correlated with lung function(P<0.05),and negatively correlated with age,smoking index.Multiple regression analysis showed that smoking index,FEV_(1) andΔFEV_(1) had a significant effect on AQP5 expression in plasma(P=0.001,P=0.000,P=0.001).The AUC of AQP5 was 0.774(P<0.001).Conclusion AQP5 has great significance in differentiating asthma,COPD and ACO.
