腹腔镜技术在低位直肠癌保肛手术中的应用价值

摘　　要：目的评估在低位直肠癌保肛手术中应用腹腔镜技术的价值。方法随机选择该院2018年2月—2020年1月期间就诊的80例直肠癌患者,以随机数表法分为对照组与观察组,每组40例,对照组接受开腹手术治疗,观察组接受腹腔镜技术治疗,对比两组低位直肠癌保肛手术患者的治疗效果。结果观察组保肛率(92.50%)高于对照组(62.50%),差异有统计学意义(χ^(2)=10.322,P=0.001);观察组术中出血量(90.23±23.25)mL低于对照组(128.45±40.53)mL、术中清扫淋巴结构数目(13.63±3.25)枚高于对照组(11.25±2.01)枚,术后疼痛消失时间(1.11±0.42)d、肠动力恢复时间(2.14±0.42)d、住院时间(1.25±0.35)d短于对照组(3.05±0.85)、(4.85±0.44)、(3.86±0.36)d,差异有统计学意义(t=5.173、3.939、12.941、28.177、32.876,P<0.001);观察组(肠梗阻、吻合口瘘、切口感染、吻合口狭窄)并发症发生率(5.00%)小于对照组(22.50%),差异有统计学意义(χ^(2)=5.164,P=0.023)。结论低位直肠癌保肛手术中应用腹腔镜技术可以取得显著成效,对于改善患者预后,提升保肛率具有重要作用。Objective To evaluate the value of laparoscopic techniques in anus-preserving surgery for low rectal cancer.Methods Randomly selected 80 patients with rectal cancer in the hospital from February 2018 to January 2020,and divided them into the control group and the observation group,40 cases each group according to the random number table.The control group received open surgery for observation.The observation group received laparoscopic technical treatment,and the treatment effect of the two groups of patients with low rectal cancer anus preservation surgery was compared.Results The anus preservation rate(92.50%)of the observation group was higher than the control group(62.50%),and the difference was statistically significant(χ^(2)=10.322,P=0.001);the intraoperative blood loss(90.23±23.25)mL of the observation group was lower than the control(128.45±40.53)mL,the number of lymphatic structures dissected during the operation(13.63±3.25)was higher than the control group(11.25±2.01),the postoperative pain disappeared time was(1.11±0.42)d,the intestinal motility recovery time was(2.14±0.42)d,hospitalization time(1.25±0.35)d shorter than the control group(3.05±0.85)d,(4.85±0.44)d,(3.86±0.36)d,and the difference was statistically significant(t=5.173,3.939,12.941,28.177,32.876,P<0.001);observation group(intestinal obstruction,anastomotic fistula complication rate of wound infection and anastomotic stenosis(5.00%)was lower than the control group(22.50%),and the difference was statistically significant(χ^(2)=5.164,P=0.023).Conclusion The application of laparoscopic technique in anus-preserving surgery for low rectal cancer can achieve significant results,which plays an important role in improving the prognosis of patients and increasing the anus-preserving rate.
