腕踝针结合双氯芬酸二乙胺乳胶剂外用治疗急性腰痛的镇痛作用研究

摘　　要：目的:观察腕踝针结合双氯芬酸二乙胺乳胶剂外用治疗急性腰痛的镇痛作用。方法:将南方医科大学南方医院白云分院收治的60例急性腰痛患者随机分为两组,分别为A组(腕踝针)和B组(腕踝针结合双氯芬酸二乙胺乳胶剂),每组30例。观察两组不同时间点的视觉模拟评分法(VAS)、重复改良的Schober试验(MMS)、Oswestry功能障碍指数问卷表(ODI)的变化,评价临床疗效。结果:(1)两组第1次治疗10、30 min及治疗后VAS评分均低于治疗前(P<0.05);B组第1次治疗30 min及治疗后VAS评分均低于A组(P<0.05)。(2)两组第1次治疗10、30 min和治疗后MMS均高于治疗前(P<0.01);两组第1次治疗10 min MMS比较差异无统计学意义(P>0.05),B组第1次治疗30 min及治疗后MMS均高于A组(P<0.05)。(3)与同组治疗前ODI比较,两组治疗后ODI均降低(P<0.01);两组间治疗后ODI比较差异有统计学意义(P<0.01)。(4)B组疗效高于A组,比较差异有统计学意义(Z=-2.354,P=0.019)。结论:腕踝针能减轻急性腰痛患者的疼痛,具有明确的即时镇痛作用,结合双氯芬酸二乙胺乳胶剂外用可使镇痛作用更好地延续。Objective:To observe the analgesic effect of wrist-ankle acupuncture combined with Diclofenac Diethylamine Emulsion for external use in the treatment of acute low back pain.Method:Sixty patients with acute low back pain admitted to the Baiyun Branch of Nanfang Hospital of Southern Medical University were randomly divided into two groups,namely the group A (wrist-ankle acupuncture) and the group B (wrist-ankle acupuncture combined with Diclofenac Diethylamine Emulsion),30 cases in each group.The changes in visual analogue scale (VAS),repeated modified Schober test (MMS),and Oswestry dysfunction index questionnaire (ODI) at different time points between the two groups were observed,and the clinical efficacy was evaluated.Result:(1) The VAS scores of the two groups at 10,30 minutes after the first treatment and after treatment were lower than those before treatment (P<0.05).The VAS scores of group B at 30 min after the first treatment and after treatment were lower than those of group A (P<0.05).(2) The MMS of the two groups at 10,30 minutes after the first treatment and after treatment were higher than that before treatment (P<0.01).There was no significant difference in MMS between the two groups at 10 minutes after the first treatment (P>0.05).The MMS of group B at 30 minutes after the first treatment and after treatment were higher than those of group A (P<0.05).(3) Compared with the ODI before treatment in the same group,the ODI after treatment in both groups decreased (P<0.01).There was significant difference in ODI between the two groups after treatment (P<0.01).(4) The curative effect of group B was higher than that of group A,and the difference was statistically significant (Z=-2.354,P=0.019).Conclusion:Wrist-ankle acupuncture can relieve the pain of patients with acute low back pain and has a clear immediate analgesic effect.The external application of Diclofenac Diethylamine Emulsion can better continue the analgesic effect.
