布地奈德鼻喷剂联合克拉霉素治疗慢性鼻窦炎的效果及对黏膜组织重塑的影响

摘　　要：目的:分析布地奈德鼻喷剂联合克拉霉素治疗慢性鼻窦炎的效果及对黏膜组织重塑的影响。方法:选取笔者所在医院耳鼻喉科2019年12月-2020年12月接诊的100例慢性鼻窦炎患者作为研究对象。根据治疗方式的不同将其分为两组,每组50例。两组均接受布地奈德鼻喷剂治疗,观察组在对照组基础上给予克拉霉素治疗。比较两组治疗前后鼻腔黏膜分泌物ECP及血清ECP水平、鼻窦断层扫描评分、治疗效果。结果:治疗前,两组鼻腔黏膜分泌物ECP和血清ECP水平比较差异均无统计学意义(P>0.05);治疗后,观察组鼻腔黏膜分泌物ECP和血清ECP水平均低于对照组,差异有统计学意义(P<0.05)。治疗后,观察组鼻窦断层扫描评分为(4.67±1.16)分,低于对照组的(11.16±1.49)分,差异有统计学意义(P<0.001)。观察组治疗总有效率为88.00%,高于对照组的56.00%,差异有统计学意义(P<0.05)。结论:布地奈德鼻喷剂联合克拉霉素治疗慢性鼻窦炎的效果优于布地奈德鼻喷剂单一药物治疗,且能够对患者鼻腔黏膜组织的重塑产生积极影响。Objective:To analyze the effect of Budesonide Nasal Spray combined with Clarithromycin in the treatment of chronic sinusitis and its effect on mucosa tissue remodeling.Method:A total of 100 patients with chronic sinusitis treated in the department of otolaryngology of our hospital from December 2019 to December 2020 were selected as the study subjects.They were divided into two groups with 50 patients in each group according to different treatment methods.Both groups were treated with Budesonide Nasal Spray,and observation group was treated with Clarithromycin on the basis of control group.The levels of nasal mucosa secretion and serum ECP before and after treatment,nasal sinus tomography score and treatment effect were compared between the two groups.Result:Before treatment,there were no significant differences in levels of nasal mucosa secretion ECP and serum ECP between the two groups (P>0.05).After treatment,the levels of nasal mucosa secretion ECP and serum ECP in the observation group were lower than those in the control group,the differences were statistically significant (P<0.05).After treatment,the nasal sinus tomography score of the observation group was (4.67±1.16) points,which was lower than (11.16±1.49) points of the control group,the difference was statistically significant (P<0.001).The total effective rate in the observation group was 88.00%,which was higher than 56.00% in the control group,and the difference was statistically significant (P<0.05).Conclusion:The efficacy of Budesonide Nasal Spray combined with Clarithromycin in the treatment of chronic sinusitis is superior to that of Budesonide Nasal Spray alone,and can positively affect the remodeling of nasal mucosa tissue in patients.
