摘 要:目的:探究与分析间歇性充气加压预防重症肺炎合并下肢深静脉血栓(DVT)形成的效果。方法:回顾性分析笔者所在医院2018年3月-2020年3月收治的重症肺炎患者100例的临床资料,按照不同处理方法分组,对照组52例,在入院之后按照指南积极进行预防性常规治疗,观察组48例,在对照组基础上加用间歇性充气加压处理。对比两组入院第4、7天的股静脉血流速度及相关凝血功能指标,同时随访1年,观察随访期间继发性DVT率。结果:入院第7天,观察组PSV、EDV、MFV、FIB值均较对照组高,D-D值较对照组低,差异有统计学意义(P<0.05);两组入院第7天的PT、INR值相比差异无统计学意义(P>0.05)。观察组与对照组随访1年继发性DVT率分别为2.08%(1/48)、15.38%(8/52),观察组低于对照组,差异有统计学意义(P<0.05)。结论:在常规预防性治疗的基础上加用间歇性充气加压处理可有效改善重症肺炎患者的股静脉血流速度及相关凝血功能指标,短期随访效果较好。Objective:To explore and analyze the effect of intermittent pneumatic compression in preventing severe pneumonia combined with deep vein thrombosis (DVT) in the lower extremities.Method:A retrospective analysis of the clinical data of 100 patients with severe pneumonia admitted to our hospital from March 2018 to March 2020.According to different treatment methods,52 cases in the control group were treated with routine preventive treatment according to the guidelines after admission,and 48 cases in the observation group were treated with intermittent pneumatic compression treatment on the basis of the control group.The femoral vein blood flow velocity and related coagulation function indexes on the 4th and 7th day after admission,and follow-up 1 years were compared,and the secondary DVT rate during the follow-up period was observed.Result:The observation group had higher PSV,EDV,MFV and FIB values 7 days after admission than the control group 7 days after admission,and the D-D value was lower,the differences were statistically significant (P<0.05).There were no significant differences in the PT and INR values of the observation group and the control group 7 days after admission (P>0.05).The observation group followed up for 1 year compared with the control group followed up for 1 year,accounted for 2.08% (1/48),15.38% (8/52),respectively,the secondary DVT rate was lower,the difference was statistically significant (P<0.05).Conclusion:On the basis of routine preventive treatment,intermittent pneumatic compression treatment can effectively improve the bone venous blood flow velocity and related coagulation function indexes in patients with severe pneumonia,and the short-term follow-up effect is better.
