伴有耳鸣耳闷的梅尼埃病个体化声治疗临床观察

摘　　要：目的观察伴有耳鸣耳闷的梅尼埃病个体化声治疗临床效果。方法伴有耳鸣耳闷症状的梅尼埃病患者102例,根据患者耳鸣响度和音调的变化进行个体化声治疗,每日1次,每次30 min,5 d一个疗程,共2个疗程。分别在治疗前及第1疗程结束时、第2疗程结束时,指导患者填写耳鸣残障量表(THI)、耳闷视觉模拟评分量表(VAS)、焦虑自评量表(SAS)、匹兹堡睡眠质量指数评分量表(PSQI),评估梅尼埃病个体化声治疗效果,包括耳鸣、耳闷、焦虑、睡眠质量,分别以耳鸣个体化声治疗有效率、耳闷治愈率、SAS评分、睡眠障碍治疗有效率表示。结果第1个疗程结束时,耳鸣个体化声治疗有效率为50%、耳闷治愈率为41%。第2个疗程结束时,耳鸣个体化声治疗有效率为92%、耳闷治愈率为82%。治疗前及第1疗程结束时、第2疗程结束时,患者SAS评分分别为(52±12)分、(44±10)分、(35±6)分,两两比较,P均<0.05。102例患者中,18例患者有睡眠障碍,第1个疗程结束时睡眠障碍治疗有效率为50%,第2个疗程结束时睡眠障碍治疗有效率为100%。结论个体化声治疗对伴有耳鸣耳闷症状的梅尼埃病患者有效,可以显著改善耳鸣耳闷症状,并缓解焦虑、改善睡眠质量,适用于不适合药物治疗或药物治疗效果不好的梅尼埃病患者。Objective To observe the clinical effects of individualized sound therapy in Meniere's disease accompa-nied with tinnitus and aural fullness.Methods Totally 102 Meniere's disease patients with tinnitus and aural fullness were treated with individualized sound therapy according to their loudness of tinnitus and changes of tonal.Each patient re-ceived 2 courses of treatment.Each course was 30 minutes once a day for 5 days,respectively.We instructed the patients to fill the Tinnitus disability scale(THI),Visual analog scale(VAS),Self-rating anxiety scale(SAS),and Pittsburgh sleep quality index(PSQI)scale before treatment and at the end of the first and second course of the treatment.The effect of individualized sound therapy in Meniere's disease was evaluated,including the tinnitus,aural fullness,anxiety and sleep quality,which were represented by the effective rate of individualized sound therapy,the cure rate of aural fullness,SAS score and the effective rate of sleep disorder respectively.Results The effective rate of individualized sound thera-py in tinnitus was 50%and the cure rate of aural fullness was 41%at the end of the first course of treatment.The effective rate of individualized sound therapy in tinnitus was 92%and the cure rate of aural fullness was 82%at the end of the sec-ond course of treatment.The SAS scores of patients were(52±12)points,(44±10)points and(35±6)points before treatment,at the end of the first course of treatment and at the end of second course of treatment respectively(all P<0.05).Among the 102 patients,18 patients suffered from sleep disorders.The effective rate of sleep disorder was 50%at the end of the first course and the effective rate of sleep order at the end of the second course was 100%.Conclusions Individualized sound therapy is effective for Meniere's disease patients with tinnitus and aural fullness,can significantly improve tinnitus and aural fullness,and relieve anxiety and improve sleep quality.It is suitable for patients with Meniere's disease who are not suitable f
