眼与心脑血管相关性及气血痰湿诊治思维

摘　　要：通过对临床高血压性视网膜病变、视网膜静脉阻塞、视网膜动脉阻塞等大量病例与血液流变学、血液动力学、心脑血管影像学等多项指标的观察,探讨视网膜动脉出现硬化的分期、分级及并发症严重程度在心脑血管系统出现变化的规律;并从气血理论论述眼底血管与气血津液相关的生理功能、血管病变与气血痰湿相关的病理变化。为进一步了解脉络瘀阻与眼底血管病变的关系以及指导用药,将血管病变转归分为四个阶段,从分期论治、辨病论治方面,以痰瘀互结为例介绍黄斑水肿的各期用药规律与体会。同时提出中医诊疗仍需传承创新,如何通过中医大数据及仿真模型可视化平台,直观展现中医的脉络瘀阻机制,是中医创新的方向,对提前预警心脑血管病变发生具有重要意义。We studied many clinical cases of hypertensive retinopathy,retinal vein occlusion and retinal artery obstruction,observed multiple indicators of blood rheology,blood dynamics and cardio-cerebrovascular imaging,to investigate the regularity of changes of retinal arteriosclerosis with different degrees of stages,grades and complications which reflected in the cardio-cerebrovascular system.We also discussed the physiological functions of the fundus blood vessels and the body fluid,and the pathological changes of the blood vessels and the phlegm and dampness based on the theory of qi and blood.In order to further understand the relationship between choroid stasis and ocular fundus vascular diseases and guide the medication,we divided the vascular disease into four stages,and introduced the medication rules and experience of each stage of macular edema from the aspects of stage treatment and disease differentiation,taking the syndrome of mutual association of phlegm and blood stasis as an example.At the same time,diagnosis and treatment of Traditional Chinese Medicine(TCM)still needs to be inherited and innovated.How to visually display the mechanism of choroid stasis in TCM through big data and visualization platform of simulation model is the direction of TCM innovation,which is of great significance for early warning the occurrence of cardiovascular and cerebrovascular diseases.
