ECPR急救流程在急诊呼吸心搏骤停患者中的应用及对自主循环恢复率的影响

摘　　要：目的探讨体外心肺复苏(ECPR)急救流程在急诊呼吸心搏骤停患者中的应用及对自主循环恢复率影响。方法回顾性分析我院急诊2018年2月至2020年5月救治的100例呼吸心搏骤停患者,其中38例仅接受传统心肺复苏(CCPR)救治,设为CCPR组,62例在CCPR救治无效后进行ECPR急救流程救治,设为ECPR组。比较两组的救治情况。结果ECPR组的自主循环恢复率、自主循环恢复48 h存活率、院内存活率均高于CCPR组,自主循环恢复时间短于CCPR组(P<0.05)。ECPR组自主循环恢复48 h的血压、血氧饱和度、心率、hs-cTnT、NT-proBNP、血乳酸水平、意识水平、意识恢复情况及神经功能恢复良好率均优于CCPR组(P<0.05)。结论ECPR急救流程应用于急诊呼吸心搏骤停患者中,能有效提高患者的自主循环恢复率和存活率,改善预后指标,效果显著。Objective To investigate the application of external cardiopulmonary resuscitation(ECPR)in emergency patients with respiratory and cardiac arrest and its effect on recovery rate of autonomous circulation.Methods A total of 100 patients with respiratory and cardiac arrest treated in our hospital from February 2018 to May 2020 were retrospectively analyzed.Among them,38 patients only received traditional cardiopulmonary resuscitation(CCPR)treatment were set as CCPR group,and 62 patients received ECPR treatment after CCPR failed were set as ECPR group.The treatment of the two groups were compared.Results The recovery rate of spontaneous circulation,48 h survival rate of spontaneous circulation recovery and in-hospital survival rate of the ECPR group were higher than those of the CCPR group,and the recovery time of spontaneous circulation was shorter than that of the CCPR group(P<0.05).The blood pressure,blood oxygen saturation,heart rate,hs-cTnT,NT-probNP,blood lactic acid level,consciousness level,consciousness recovery and good rate of neurological function recovery at 48 h after spontaneous circulation recovery in the ECPR group were better than those in the CCPR group(P<0.05).Conclusion The application of ECPR emergency procedure in emergency patients with respiratory and cardiac arrest can effectively increase the recovery rate of spontaneous circulation and survival rate of patients,and improve the prognosis indexes,with significant effect.
