汪文绮《杂症会心录》疫病辨治特色探析

摘　　要：清代新安医家汪文绮晚年著作《杂症会心录》汇集了汪氏数十年的临证经验,书中对于疾病的辨证析因细致并列医方、医案于后。通过与吴又可、叶天士等医家观点方法的比较,发现汪文绮对疫病的论述颇具特色。其在《杂症会心录》中深入探讨并发挥了“邪从口鼻而入”之论,在继承基础上阐明了“邪陷三阴”的疫病病机,注重辨明正气虚实,针对疫邪热毒伤阴的特点创立救阴结合解疫毒的治疫大法,创制有乾一老人汤、新制救疫汤两首治疫代表方剂,善用豆类与甘草之类配伍扶正解毒,用药具有“平正轻简”的特点,其解疫毒的同时重视固护阴液、变通活法而不拘泥于古的思想,对现代疫病理论的完善和临床治疗方案的选择有重要的参考价值和现实指导意义。WANG Wen-qi was a doctor of Xin’an school in Qing Dynasty.He wrote a book,Za Zheng Hui Xin Lu,which collected his clinical experience.The differentiation and cause analysis of diseases were detailed,and the medical prescriptions and records were listed in the book.Comparing with WU You-ke and YE Tian-shi,we found that WANG Wen-qi had a unique view on epidemic diseases.In Za Zheng Hui Xin Lu,he deeply discussed and developed the theory of“evil coming from the mouth and nose”,clarified the pathogenesis of epidemic disease from"evil invading three Yin",and emphasized the deficiency and excess of Qi.According to the characteristics of pathogenic evil,he created the method of treating epidemic diseases by saving Yin combined with detoxification.Then he invented two representative prescriptions,Qianyi Leoren Decoction and Xinzhi Jiuyi Decoction.In addition,he was good at using"calmness,light and simplicity"herbs,such as beans and licorice.WANG Wen-qi’s academic thoughts included paying attention to strengthen Yin fluid while clearing toxin,flexible and not rigidly adhere to the ancients,which is of great value to the improvement of modern epidemic theory and the choice of clinical treatment plan.
