摘 要:目的:通过比较电针不同部位配伍腧穴对胰岛素抵抗肥胖大鼠肠道屏障功能及肠道炎症状态的影响,探讨“标本配穴”治疗胰岛素抵抗肥胖的效果及机制。方法:60只Wistar大鼠按随机数字表法分为正常组(A组)、模型组(B组)、腹部电针干预组(C组)、下肢电针干预组(D组)和标本配穴电针干预组(E组)每组各8只,采用高脂饲料喂养建立胰岛素抵抗肥胖大鼠模型,A、B组不做干预,C组选取“关元”和“中脘”,D组选取“足三里”和“丰隆”,E组选取“中脘”“关元”“足三里”“丰隆”4穴进行电针治疗,分组干预治疗8周。治疗前后测量大鼠体质量、血糖水平及葡萄糖输注速率(glucose infusion rate,GIR),分别采用免疫印迹法及实时荧光定量PCR法检测结肠中闭锁连接蛋白-1(zonula occludens-1,ZO-1)、紧密连接蛋白(Occludin)、肿瘤坏死因子α(tumor necrosis factor,TNF-α)、白细胞介素1β(interleukin 1β,IL-1β)蛋白和基因的表达。结果:治疗前与A组比较,B、C、D和E组大鼠体质量、餐后血糖明显升高(P<0.01,P<0.05),GIR明显降低(P<0.01)。治疗结束后与A组比较,B组大鼠体质量、空腹血糖、餐后血糖明显升高(P<0.01),GIR明显降低(P<0.01),ZO-1、Occludin基因与蛋白的表达均明显降低(P<0.01),TNF-α、IL-1β蛋白与mRNA的表达均明显升高(P<0.01,P<0.05);与B组比较C、D与E组大鼠的体质量、餐后血糖明显降低(P<0.05,P<0.01),GIR明显升高(P<0.05,P<0.01),C组和E组ZO-1的蛋白水平明显升高(P<0.05,P<0.01),C组、D组和E组Occludin蛋白水平均明显升高(P<0.01),E组ZO-1和Occludin的mRNA水平明显升高(P<0.05),E组大鼠TNF-α、IL-1β蛋白与mRNA水平均显著降低(P<0.01);C组和D组TNF-α、IL-1β蛋白水平明显降低(P<0.05,P<0.01);与E组比较,C组和D组的Occludin蛋白水平降低(P<0.05),TNF-α、IL-1β蛋白水平升高(P<0.01)。结论:电针治疗能够改善胰岛素抵抗肥胖大鼠肠道屏障功能及肠道炎症状�Objective:To study the mechanism of the different acupoints compatibility on the function of intestinal barrier and inflammatory state by in insulin resistant obese rats.Methods:60 Wistar rats were divided into A group(8 normal rats),feed by normal forage and model group(52 rats),feed by high-fat forage.After modeling,32 insulin resistance obesity rats were randomly divided into 4 groups,8 rats in each group,group B(Model group),group C(abdominal electroacupuncture group),group D(lower limbs electroacupuncture group),and group E(Biaoben electroacupuncture group).Group A and group B did not intervene.group C selected Guanyuan(RN4)and Zhongwan(RN12),group D selected Zusanli(ST36)and Fenglong(ST40),and group E selected RN12,RN4,ST36 and ST40 for electroacupuncture treatment.The body weight,glucose level and glucose infusion rate were measured before and after treatment.Western blotting and Real-time fluorescent quantitative-PCR were used to detect the expression of ZO-1,occludin,TNF-α,and IL-1βin colon.Results:After treatment,the body weight,fasting blood glucose and postprandial blood glucose of group B were significantly higher than those of group A,and GIR was decreased(P<0.01);Compared with group B,the body weight and postprandial blood glucose were significantly lower after acupuncture intervention(P<0.05);Compared with group A,the expressions of ZO-1,occludin gene and protein in group B was significantly lower(P<0.05);compared with group B,the expression of ZO-1,occludin gene and protein was significantly higher after acupuncture intervention,especially in Group E(P<0.05).Compared with group A,the expressions of TNF-α,IL-1βgene and protein in group B was significantly higher(P<0.05);Compared with group B,the expression of TNF-α,IL-1βgene and protein in electroacupuncture intervention groups decreased,especially in Group E(P<0.05).Conclusion:Electroacupuncture can improve the intestinal barrier function and intestinal inflammation state of insulin resistant obese rats,and the effect of Biaoben acupoint el
