补肾活血汤治疗股骨头坏死的临床疗效观察

摘　　要：目的探讨补肾活血汤在股骨头坏死治疗中的应用效果。方法72例股骨头坏死患者,按照系统抽样法分为对照组与观察组,每组36例。对照组患者采用健骨生丸治疗,观察组患者采用补肾活血汤治疗。对比两组治疗效果,血液流变学指标,髋关节活动度和髋关节疼痛评分,软骨缺损评分。结果观察组治疗总有效率91.67%高于对照组的72.22%,差异具有统计学意义(P<0.05)。治疗后,观察组全血低切粘度、全血高切粘度分别为(8.69±0.92)、(4.10±1.16)mPa·s,均低于对照组的(9.27±1.10)、(5.10±0.65)mPa·s,差异具有统计学意义(P<0.05)。治疗后,观察组髋关节活动度和髋关节疼痛评分分别为(7.84±2.32)、(7.53±2.11)分,均高于对照组的(5.65±1.86)、(5.43±1.75)分,差异具有统计学意义(P<0.05)。治疗后,观察组软骨缺损评分为(2.95±0.92)分,低于对照组的(3.41±1.03)分,差异具有统计学意义(t=1.998,P=0.049<0.05)。结论股骨头坏死患者采用补肾活血汤治疗不仅可以增强患者髋关节功能,减轻髋部疼痛,而且有助于血液流变学指标改善,确保良好的治疗效果,值得临床进一步采纳与推广。Objective To discuss the practical effect of Bushen Huoxue Decoction in the treatment of femur head necrosis.Methods A total of 72 patients with femur head necrosis were divided into control group and observation group according to systematic sampling method,with 36 cases in each group.The control group was treated with Jiangusheng Pills,and the observation group was treated with Bushen Huoxue Decoction.The therapeutic effect,hemorheological indexes,hip range of motion and hip pain score,and cartilage defect score were compared between the two groups.Results The total effective rate 91.67%of the observation group was higher than 72.22%of the control group,and the difference was statistically significant(P<0.05).After treatment,the whole blood low-shear viscosity and whole blood high-shear viscosity of the observation group were(8.69±0.92)and(4.10±1.16)mPa·s,which were lower than(9.27±1.10)and(5.10±0.65)mPa·s of the control group,and the difference was statistically significant(P<0.05).After treatment,the hip range of motion and hip pain score of the observation group were(7.84±2.32)and(7.53±2.11)points,which were all higher than(5.65±1.86)and(5.43±1.75)points of the control group,and the difference was statistically significant(P<0.05).After treatment,the cartilage defect score of the observation group was(2.95±0.92)points,which was lower than(3.41±1.03)points of the control group,and the difference was statistically significant(t=1.998,P=0.049<0.05).Conclusion In the clinical treatment of femur head necrosis,Bushen Huoxue Decoction can not only enhance the hip joint function,reduce hip pain,but also help to improve hemorheological indexes,ensure good treatment effect,which is worthy of further adoption and promotion.
