摘 要:目的研究切开复位内固定术与人工肱骨头置换术治疗肱骨近端骨折的效果及安全性。方法100例肱骨近端骨折患者,按随机数字表法分为观察组和对照组,各50例。观察组接受切开复位内固定术治疗,对照组接受人工肱骨头置换术治疗。比较两组患者治疗效果、手术情况、术后并发症发生情况。结果观察组治疗总有效率94.00%显著优于对照组的76.00%,差异具有统计学意义(P<0.05)。观察组手术时间(68.13±7.41)min、骨折愈合时间(16.32±3.05)周均显著短于对照组的(90.22±10.36)min、(20.13±4.50)周,术中出血量(185.02±22.98)ml以及术后引流量(50.37±10.27)ml均显著少于对照组的(241.34±27.69)、(74.82±15.30)ml,差异具有统计学意义(P<0.05)。两组术后并发症发生率比较差异无统计学意义(P>0.05)。结论肱骨近端骨折采用切开复位内固定术的疗效比人工肱骨头置换术更佳,值得临床应用。Objective To study the effect and safety of open reduction and internal fixation and artificial humeral head replacement in the treatment of proximal humeral fractures.Methods A total of 100 patients with proximal humeral fractures were divided into observation group and control group according to random numerical table,with 50 cases in each group.The observation group was treated with open reduction and internal fixation,and the control group was treated with artificial humeral head replacement.The therapeutic effect,surgical status,and occurrence of postoperative complications were compared between the two groups.Results The total effective rate 94.00%of the observation group was significantly better than 76.00%of the control group,and the difference was statistically significant(P<0.05).The operation time(68.13±7.41)min and fracture healing time(16.32±3.05)weeks of the observation group were significantly shorter than(90.22±10.36)min and(20.13±4.50)weeks of the control group,and the intaroperative blood loss(185.02±22.98)ml and postoperative drainage volume(50.37±10.27)ml were significantly less than(241.34±27.69)and(74.82±15.30)ml of the control group.All the difference was statistically significant(P<0.05).There was no statistically significant difference in incidence of postoperative complications between the two groups(P>0.05).Conclusion Open reduction and internal fixation has better efficacy than artificial humeral head replacement in the treatment of proximal humeral fractures,and it is worthy of clinical application.
