吡格列酮辅助炔雌醇环丙孕酮治疗多囊卵巢综合征的效果

摘　　要：目的观察吡格列酮辅助炔雌醇环丙孕酮治疗多囊卵巢综合征(PCOS)的效果。方法选取2018年7月至2019年12月平顶山市第五人民医院收治的91例PCOS患者,按照随机数字表法分为对照组(45例)和联合组(46例)。对照组接受炔雌醇环丙孕酮治疗,联合组接受吡格列酮辅助炔雌醇环丙孕酮治疗。比较两组治疗前后性激素[黄体生成素(LH)、睾酮(T)、卵泡刺激素(FSH)]水平、胰岛功能指标[空腹血糖(FBG),空腹胰岛素(FINS)、稳态模型评估的胰岛素抵抗指数(HOMA-IR)]水平以及子宫内膜胰岛素受体表达情况。结果治疗后,两组血清T、FSH、LH水平均较治疗前降低,且联合组血清T、FSH、LH水平均低于对照组(P<0.05)。治疗后,两组FBG、FINS、HOMA-IR水平均较治疗前降低,且联合组FBG、FINS、HOMA-IR水平均低于对照组(P<0.05)。治疗后,联合组子宫内膜异常增生患者占比低于对照组(P<0.05),且联合组子宫内膜异常增生患者子宫内膜腺体细胞胰岛素受体阴性占比高于对照组(P<0.05)。结论吡格列酮辅助炔雌醇环丙孕酮治疗PCOS患者,可改善患者性激素水平,提高胰岛功能,且可改变子宫内膜胰岛素受体异常表达。Objective To observe the effect of pioglitazone assisted ethinylestradiol and cyproterone acetate in the treatment of polycystic ovary syndrome(PCOS).Methods A total of 91 patients with PCOS who were treated in the Fifth People’s Hospital of Pingdingshan from July 2018 to December 2019 were divided into control group(45 cases)and combination group(46 cases)according to the random number table method.The control group accepted ethinylestradiol and cyproterone acetate,and the combination group accepted pioglitazone assisted ethinylestradiol and cyproterone acetate.The levels of sex hormone[luteinizing hormone(LH),testosterone(T),follicle stimulating hormone(FSH)],pancreatic islet function indexes[fasting blood glucose(FBG),fasting insulin(FINS),homeostasis model assessment-insulin resistance index(HOMA-IR)]and endometrial insulin receptor expression before and after treatment were compared between the two groups.Results After treatment,the levels of serum T,FSH and LH of the two groups were lower than those of before treatment,and the levels of serum T,FSH and LH of the combination group were lower than those of the control group(P<0.05).After treatment,the levels of FBG,FINS and HOMA-IR of the two groups were lower than those of before treatment,and the levels of FBG,FINS and HOMA-IR of the combination group were lower than those control group(P<0.05).After treatment,the proportion of patients with endometrial hyperplasia of the combination group was lower than that of the control group,and the proportion of endometrial glandular cells with negative insulin receptor in patients with endometrial hyperplasia of the combination group was higher than that of the control group(P<0.05).Conclusion Pioglitazone assisted ethinylestradiol and cyproterone acetate can effectively improve the levels of sex hormone of pcos patients and pancreatic islet function and change the abnormal expression of endometrial insulin receptor.
