探讨基于Autar量表的护理干预对妊娠期高血压疾病剖宫产产妇血流动力学和下肢静脉血栓形成的影响

摘　　要：目的探讨基于Autar量表的护理干预对妊娠期高血压疾病剖宫产产妇血流动力学和下肢静脉血栓形成的影响。方法选择我院2015年1月至2016年12月收治的100例妊娠期高血压疾病剖宫产产妇作为研究对象,根据随机原则进行分组,每组50例,对照组采用常规护理措施,观察组基于对照组护理措施,给予产妇基于Autar量表的护理干预,比较两组患者的生活质量评分、血流动力学变化、静脉血流速度。结果两组患者护理后生活质量评分明显比治疗前提高,且观察组护理后生活质量评分明显高于对照组,P<0.05。两组患者术前的心率、DBP、SBP无明显差异,P>0.05;两组患者麻醉后5 min、术后24 h的心率、DBP、SBP明显比术前降低,术后24 h的心率、DBP、SBP明显比麻醉后5 min升高,且观察组麻醉后5 min、术后24 h的心率、DBP、SBP明显比对照组更低,P<0.05。观察组患者的腘静脉、股静脉血流速度明显比对照组更快,P<0.05。结论妊娠期高血压疾病剖宫产产妇采用基于Autar量表的护理干预,有利于促进产妇的下肢血液运行状况,可以有效地改善产妇的血流动力学,有利于预防下肢深静脉血栓的形成。Objective To explore the effect of nursing intervention based on Autar scale on hemodynamics and venous thrombosis of lower limbs in pregnant women with hypertensive cesarean section.Methods From January 2015 to December 2016,100 cases of cesarean section maternal gestational hypertension disease as the research object,according to the principle of random grouping,each group of 50 cases,control group using conventional nursing measures,observation group based on the control group nursing measures,gives the maternal Autar scale based nursing intervention,compared two groups of patients'quality of life scores,hemodynamic changes,venous blood flow velocity.Results The quality of life score after nursing was significantly higher than before treatment in the two groups,P<0.05;the life quality score of observation group was significantly higher than that of control group,P<0.05.There was no significant difference in preoperative heart rate,DBP and SBP between the two groups(P>0.05).The heart rate,DBP and SBP of the two groups were significantly lower at 5 min after anesthesia and 24 h after surgery than before surgery,while the heart rate,DBP and SBP were significantly higher at 24 h after surgery than 5 min after anesthesia,the heart rate,DBP and SBP of the observation group were significantly lower than those of the control group 5 min after anesthesia and 24 h after surgery(P<0.05).The flow velocity of popliteal vein and femoral vein in the observation group was significantly faster than that in the control group(P<0.05).Conclusion Nursing intervention based on Autar scale in pregnant women with hypertensive cesarean section is beneficial to promote the blood movement of lower limbs,effectively improve the hemodynamics of puerpera,and prevent the formation of deep venous thrombosis of lower limbs.
