摘　　要：目的探讨应用垂直疤痕乳房缩小术修复双环法乳房缩小术后畸形的可行性。方法回顾分析2018年1月~2021年3月期间,应用垂直疤痕乳房缩小术修复的双环法乳房缩小术后形态不满意的患者,对其治疗结果及并发症进行统计。结果期间共诊治患者38例共76侧乳房,均为双环法乳房缩小术后,双侧乳房畸形。表现为乳房形态扁平,部分伴有乳晕周围瘢痕宽大,乳晕直径增大。有三侧乳房发生乳头坏死,21例患者42侧乳房在双环法乳房缩小的同时放置乳房假体。有11例患者22侧乳房双环法缩小术后进行过自体脂肪移植。所有患者均采用垂直疤痕乳房缩小术塑形的原则进行修复。其中6例患者修复的同时进一步行乳房缩小。术后形态均得到明显改善,无感染、乳头乳晕坏死等并发症的发生。结论传统双环法乳房缩小手术具有一定的不足,正确理解手术原则,采用垂直疤痕乳房缩小塑形的方法可以有效纠正术后畸形。Objective Explore the application of vertical scar breast reduction in the patients who are not satisfied after periareolar reduction mammaplasty.Methods From January 2018 to March 2021,patients who were repaired deformity by vertical scar breast reduction after peri-areolar reduction mammaplasty were retrospectively analyzed.Results 38 patients with 76 sides of the breast deformities after peri-areolar reduction mammaplasty were included.The deformity is characterized by flat breast,wide scar,and enlarged areola.Three of them occurred areola necrosis.All patients were treated by vertical scar breast reduction mammaplasty.All the appearance of the breast improved significantly after surgery,and no complications occurred.Conclusion The traditional periareolar reduction mammaplasty has some shortcomings.The deformity after that can be effectively corrected by the vertical scar breast reduction method.
