内上方真皮腺体蒂技术在多种乳房畸形整复中的临床应用

摘　　要：目的探讨内上方真皮腺体蒂技术在多种乳房畸形整复中的临床应用及效果。方法回顾分析我科采用内上方真皮腺体蒂技术在巨乳症、乳房下垂、假体取出后Ⅰ期乳房塑形、聚丙烯酰胺水凝胶取出术后Ⅰ期乳房塑形等多种乳房畸形整复中的应用效果。结果共治疗患者216例,其中巨乳症患者150例,乳房肥大松垂(外观以松垂为主)患者46例,假体取出后Ⅰ期乳房塑形9例,聚丙烯酰胺水凝胶取出术后Ⅰ期乳房塑形患者11例;术后巨乳症患者乳房下皱襞处血清肿者5人,经穿刺抽吸及加压包扎治疗后痊愈,乳房下皱襞处伤口小段裂开者3例,经换药后再次缝合治愈,86例患者随访6~18个月(平均12个月),乳头乳晕复合体血运及形态良好,均未发现有乳头乳晕坏死及感觉障碍;其中77例患者对手术效果满意。结论内上方真皮腺体蒂技术是一种安全有效的治疗各种乳房畸形的方法,能有效保障新乳头乳晕复合体的血供及感觉功能,远期乳房塑形效果良好。Objective To summarize the clinical application and effect of the superomedial pedicle(SNP)mammoplasty technique in various kinds of breast deformity in the past ten years.Methods A retrospective study of all patients who underwent mammoplasty with SMP technique,including gigantomastia,mastoptosis,one stage breast shaping after removal of prosthesis and Polyacrylamide hydrogel.Results A total of 105 patients were treated,including 62 patients with macromastia,23 patients with mastoptosis,9 patients with one stage breast shaping after removal of prosthesis,and 11 patients with one stage breast shaping after removal of Polyacrylamide hydrogel.Postoperative 5 patients with macromastia ouccerd seroma,they were cured after puncture and pressure dressing;3 patients with part of the wound opened,and it was cured by suturing again after dressing change.86 patients were followed up for 6-18 months(mean 12 months),the blood supply and morphology of nipple areola complex were good,and no necrosis of nipple areola and sensory disturbance were found;77 patients were satisfied with the operation.Conclusion SNP technique is a safe and effective technique for the treatment of breast hypertrophy,the risk of nipple areola complex necrosis is low,the feeling of areola areola complex and the long-term breast shaping effect is good.
