摘  要:目的探讨胫骨横向骨搬移技术对糖尿病兔下肢溃疡创面促细胞趋化水平的影响。方法SD级雄性新西兰白兔32只,8月龄,重量2~2.5kg。将兔子通过数字表法随机分成四组(n=8):正常兔+假手术组(A)、正常兔+胫骨横向骨搬移治疗组(B)、糖尿病兔+假手术组(C)、糖尿病兔+胫骨横向骨搬移治疗组(D)。通过静脉注射链脲佐菌素(STZ)法建立糖尿病兔模型,均予以结扎大腿的股动脉,均在同侧足背上做2cm×4cm大小的创面。术后立即开始骨搬移,一天搬移三次,约0.3mm/天,术后6天内向外进行骨搬移,6天后按同样的速度搬回骨块,每3天摄次片观察搬移情况。假手术组只有切口和组织剥离的程度与骨搬移手术组一致。四组在(术前,术后3天、6天、9天、12天)收集创面组织,用酶联免疫吸附法测定同一时间段创面组织因子的表达水平:粒细胞-巨噬细胞集落刺激因子(GM-CSF)、转化生长因子-β1(TGF-β1)。结果在动物实验中,糖尿病溃疡创面能抑制组织因子(GM-CSF、TGF-β1)的表达(P<0.05),胫骨横向骨搬移能促进糖尿病溃疡创面中组织因子(GM-CSF、TGF-β1)的表达(P<0.05)。结论在动物实验中,糖尿病溃疡创面能抑制组织因子(GM-CSF、TGF-β1)的表达,胫骨横向骨搬移能促进糖尿病溃疡创面中组织因子(GM-CSF、TGF-β1)的表达,从而促进糖尿病溃疡创面的趋化多种细胞的能力。Objective To investigate the effect of transverse bone transfer technique of tibia on the level of cell chemotaxis induced by lower extremity ulcer in diabetic rabbits Methods 32 male SD New Zealand white rabbits,8 months old,with body mass 2~2.5kg.Rabbits were randomly divided into four groups(n=8)by digital table method:normal rabbit+sham operation group(A),normal rabbit+transverse bone transfer technique of tibia group(B),diabetic rabbit+sham operation group(C),diabetes Rabbit+transverse bone transfer technique of tibia group(D).Diabetic rabbit model was established by intravenous injection of streptozotocin(STZ).The femoral artery of the same side was ligated,and the wounds of 2cm×4cm were made on the ipsilateral foot.Immediately after the operation,the bone movement was started,three times a day,about 0.3 mm/day,and the bone was moved outward within 6 days after the operation.After 6 days,the bones were moved back at the same speed,and the movement was observed every 3 days.In the sham operation group,only the degree of incision and tissue dissection was consistent with the bone transfer surgery group.Wound tissue was collected in four groups(preoperative,3 days,6 days,9 days,12 days after surgery),and the expression level of tissue factor in wounds was measured by enzyme-linked immunosorbent assay:granulocyte-macrophage colony stimulation Factor(GM-CSF),transforming growth factor β1(TGF-β1)Results In animal experiments,diabetic ulcer wounds can inhibit the expression of tissue factor(GM-CSF,TGF-β1)(P<0.05),and Transverse tibial bone transfer can promote the expression of tissue factor(GM-CSF,TGF-β1)(P<0.05)Conclusion In animal experiments,diabetic ulcer wounds can inhibit the expression of tissue factors(GM-CSF,TGF-β1),and transverse tibial bone movement can promote the expression of tissue factors(GM-CSF,TGF-β1)in diabetic ulcer wounds,thus promoting the chemotactic ability of multiple cells in diabetic ulcer wounds.
