摘　　要：目的总结颌面部骨纤维异常增殖行局部骨骼切削塑形术患者的围术期护理措施,为临床规范护理策略的制定提供借鉴。方法对2016年5月~2020年10月在我院接受局部骨骼切削塑形术的72例颌面部骨纤维异常增殖症患者实施健康宣教、心理干预、营养评估、术前准备及呼吸放松训练、术中护理、术后疼痛护理、创面观察及并发症预防护理、康复训练、出院指导等系统化围手术期护理。结果72例患者均顺利完成局部骨骼切削塑形术,面部不对称、眼球移位、视力减退、张口困难等症状均明显改善;术后发生局部出血3例,应用止血药物控制;1例出现面部软组织下垂,经面部悬吊术后纠正;所有患者手术创面均为甲级愈合,无明显瘢痕及色素沉,面部对称良好,患者对塑形效果满意,护理满意度为100%。结论局部骨骼切削塑形术是治疗颌面部骨纤维异常增殖症的首选手段,系统围手术期护理是保证手术顺利完成,降低术后并发症,提升颌面部塑形效果的重要保障措施。Objective Summarize the perioperative nursing measures for patients with abnormal proliferation of maxillofacial bone fibers undergoing local bone ablation and plastic surgery,and provide a reference for the formulation of clinical standardized nursing strategies.Methods From May 2016 to October 2020,72 patients with maxillofacial bone fibrous dysplasia who underwent local bone cutting and shaping in our hospital were provided with health education,psychological intervention,nutritional assessment,preoperative preparation and breathing relaxation training,Intraoperative nursing,postoperative pain nursing,wound observation and complication preventive nursing,rehabilitation training,discharge guidance and other systematic perioperative nursing.Results All 72 patients successfully completed the local bone reshaping surgery,and the symptoms of facial asymmetry,eyeball shift,vision loss,difficulty in opening the mouth and other symptoms were significantly improved;3 cases of local bleeding occurred after operation,which was controlled by hemostatic drugs;1 case developed facial soft tissue The sagging was corrected after facial suspension;all the surgical wounds of the patients were healed at grade A,there was no obvious scar and pigmentation,and the facial symmetry was good.The patients were satisfied with the shaping effect,and the nursing satisfaction was 100%.Conclusion Local bone ablation and shaping is the first choice for the treatment of maxillofacial bone fibrous dysplasia.Systematic perioperative care is an important guarantee to ensure the smooth completion of the operation,reduce postoperative complications,and improve the effect of maxillofacial shaping.
