中西医结合疗法对肺门淋巴结核临床疗效

摘　　要：目的:探讨中西医结合疗法治疗肺门淋巴结核的临床疗效。方法:回顾性分析河北省胸科医院及衡水市三院2018-2020年期间收治的经超声支气管镜引导下穿刺活检术(EBUS-TBNA)确诊的肺门淋巴结核患者60例,随机分为中西医结合组及对照组各30例。对照组为常规抗结核治疗药物,中西医结合组为对照组基础上联合消瘰散结散外用后经中医定向透药及月华丸口服。2个月为1疗程,观察两组治疗1个疗程后临床疗效、中医证候评分、不良反应及随访8个月后治愈率、中医证候缓解率。结果:肺门淋巴结核经中西医结合组治疗后有效率96.7%明显高于对照组76.7%(χ^(2)=5.192,P=0.023)。中西医结合组较对照组能明显改善中医证候及治愈率(P<0.05)。结论:经EBUS-TBNA确诊肺门淋巴结核后及时采取中西结合治疗方案较单纯抗结核治疗方案能提高有效率、缓解中医证候、提高后续疗效且安全性高。Objective The clinical curative effect of integrated traditional Chinese and Western medicine was explored in 30 cases of hilar lymphatic tuberculosis.Methods A retrospective analysis was performed in 60 patients with hilar lymphatic tuberculosis diagnosed by ultrasound bronchoscopy-guided needle biopsy(EBUS-TBNA)admitted to Hebei Chest Hospital and the Third Hospital of Hengshui from 2018 to 2020.The patients were randomly divided into Chinese and Western medicine combination group and control group.The control group was treated with conventional anti-tuberculosis drugs,and the integrated traditional Chinese and Western medicine group was used as based on the control group with the combination of Xiaoluo Sanjie San and oral administration of Chinese medicine directed drug penetration and Yuehua pills.Two months was a course of treatment.The clinical efficacy,TCM syndrome scores,adverse reactions,cure rate and TCM syndrome remission rate after eight months of follow-up were observed after one course of treatment in two groups.Results The effective rate of hilar lymphatic tuberculosis treated with integrated traditional Chinese and western medicine was 96.7%,which was significantly higher than that of control group 76.7%(χ^(2)=5.192,P=0.023).The integrated traditional Chinese and Western medicine group significantly improved TCM syndromes and cure rateas compared with control group(P<0.05).Conclusions After the diagnosis of hilar lymphatic tuberculosis by EBUS-TBNA,timely combined Chinese and Western treatment can improve the effective rate,alleviate the symptoms of TCM,and improve the follow-up efficacy and safety compared with pure anti-tuberculosis treatment.
