2型糖尿病合并代谢相关脂肪性肝病患者胎球蛋白A水平与胰岛素抵抗的相关性研究

摘　　要：目的观察T2DM合并代谢相关脂肪性肝病(MAFLD)患者胎球蛋白A(FA)水平,并分析其与IR的相关性。方法选取2018年11月至2020年10月于我院新诊断的单纯T2DM患者(T2DM组)、单纯MAFLD患者(MAFLD组)和T2DM合并MAFLD患者(T2DM+MAFLD组),每组各160例,另选取同期体检健康人群(NC组)160名。检测各组血清FA水平,分析FA与各指标的相关性。结果 T2DM+MAFLD、T2DM、MAFLD及NC组血清FA水平依次降低(P<0.05);T2DM+MAFLD组FA水平与BMI(r=0.813,P<0.001)、TG(r=0.637,P<0.001)、FPG(r=0.583,P<0.001)、2 hPG(r=0.681,P<0.001)、HbA1c(r=0.581,P<0.001)和胰岛素抵抗指数(HOMA-IR)(r=0.735,P<0.001)呈正相关,与HDL-C(r=-0.219,P=0.005)、胰岛素分泌指数(HOMA-IS)(r=-0.693,P<0.001)呈负相关。结论血清FA水平升高与HOMA-IR是T2DM合并MAFLD患者的危险因素,HOMA-IS是T2DM合并MAFLD患者的保护因素。Objective To observe fetuin-A(FA)level in patients with type 2 diabetes mellitus(T2 DM)complicated with metabolic associated fatty liver disease(MAFLD)and analyze its correlation with insulin resistance(IR).Methods From Nov. 2018 to Oct. 2020,patients with T2 DM(T2 DM group),MAFLD(MAFLD group),the combination of the two(T2 DM+MAFLD group),and healthy controls(NC group)were recruited,with 160 cases in each group. The levels of serum FA were detected.The correlations between FA and other indicators were analyzed.Results The serum FA levels of T2 DM+MAFLD group,T2 DMgroup,MAFLD group and NC group decreased successively(P<0. 05).In T2 DM + MAFLD group,FA level was positively correlated with BMI(r=0. 813,P<0. 001),TG(r=0. 637,P<0. 001),FPG(r=0. 583,P<0. 001),2 h PG(r=0. 681,P<0. 001),HbA1c(r=0. 581,P<0. 001)and HOMA-IR(r=0. 735,P<0. 001),and negatively correlated with HDL-C(r=-0. 219,P=0. 005),HOMA-IS(r=-0. 693,P<0. 001).Conclusion Serum FA level was positively correlated with IR,both of which are risk factors for T2 DM + MAFLD. HOMA-IS was the protective factor of T2 DM+MAFLD.
