韦贵康运用手法配合中药内服外用治疗早中期股骨头缺血性坏死经验

摘　　要：总结韦贵康教授治疗早中期股骨头缺血性坏死的手法特点和用药特点。股骨头缺血性坏死呈渐进病理过程,临证时需结合患者的证候特点和病理发展进程,遵循"顺生理、反病理"的治则,以"通"为用为治疗根本,手法与中药内服外用相结合,内外兼顾,筋骨并重。手法治疗以"理筋、调骨、对症"三联疗法为基础。内服中药注重分期辨证,早期湿热多见,以清热祛湿、活血通脉为主;中期多痰瘀互结,加之久病肝肾亏虚,筋骨失养,治以化痰祛瘀、补益肝肾。同时采用具有舒筋活络、活血止痛功效的中药煎汤湿敷。This paper summarized professor WEI Guikang′s manipulation and medication experience in treating early and mid-stage avascular necrosis of the femoral head.As avascular necrosis of the femoral head is complex and gradually progressed,it is necessary to consider the patient′s syndromes and pathological process in clinical practice,follow the principle of"obeying physiological characteristics,and reversing pathological changes",take"unblocked transmission"of the normal function as the core,combine manipulation therapy with orally and externally administered herbal medicine,treat both the interior and exterior,and lay emphasis on both sinews and bones.Manipulation is mainly based on the triple therapy of"tendons-regulating,bone-setting,and symptomatic intervention".Syndrome differentiation and staged prescription of oral herbal medicine are recommended;at early stage when damp-heat syndrome is commonly seen,it is better to clear heat and dispel dampness,invigorate blood and unblock the channels;at middle stage when binding of phlegm and stasis is the major syndrome,and is accompanied by depletion of liver-kidney and failure of nourishing sinew-bone due to chronic and long illness,it is advised to dissolve phlegm and dispel stasis,supplement and boost the liver and kidney.Simultaneously,herbal medicine which can unblock the channels and quicken the collaterals,invigorate blood and relieve pain are suggested to be dococted and externally applied.
