摘  要:目的探讨成年男性γ-谷氨酰转肽酶和肌酐与高尿酸血症(HUA)之间的相关性。方法分析735例体检的成年男性HUA患病情况及其影响因素。采用logistic回归分析构建3种模型评估调整不同变量情况下不同GGT和肌酐水平的四分位分组与HUA发生风险的相关性。其中,模型1未调整其他变量,模型2调整了年龄、BMI、基础代谢率(BMR)、吸烟、饮酒和睡眠因素,模型3在模型2的基础上调整了肌酐或GGT、代谢综合征、冠心病、脑卒中、HDL、LDL、TG及TC。采用限制性立方样条方法,绘制GGT和肌酐水平与HUA发生风险的剂量-反应关系。结果 735例成年男性中,192例(26.12%)患HUA。HUA组和非HUA组的年龄、睡眠、BMI、BMR、GGT、肌酐、TG以及代谢综合征、冠心病和脑卒中发生率比较均存在统计学差异(P<0.05)。GGT和肌酐水平与尿酸水平呈正相关(P<0.05)。模型1、2、3分析结果均显示,以GGT<22 U/L作为参照,GGT 22 U/L~、GGT 30 U/L~和GGT>44 U/L为HUA发生的危险因素(P<0.01);以肌酐<67 mmol/L作为参照,肌酐>81 mmol/L为HUA发生的危险因素(P<0.01)。肌酐和GGT水平与HUA发生风险均呈正向线性剂量-反应关系。GGT与肌酐对HUA发生风险的影响存在一定交互作用,当GGT与肌酐均为高水平时(即GGT>44 U/L、肌酐>81 mmol/L),HUA的发生风险最高(P<0.01)。结论男性GGT和肌酐水平升高可能是HUA发生、发展的危险因素。Objective To explore the correlation between gamma-glutamyl transpeptidase(GGT),creatinine and hyperuricemia(HUA) in male population.Methods The prevalence of HUA and its influencing factors were analyzed in 735 adult men underwent physical examination.A logistic regression analysis was used to construct three models to evaluate the association between interquartile groups of different GGT and creatinine levels and the risk for HUA under the condition of adjusting different variables.Model 1 did not adjust other variables, model 2 adjusted for age, BMI,and basal metabolism, and model 3 adjusted for creatinine or GGT,metabolic syndrome, coronary heart disease, stroke, HDL,LDL,TG and TC on the basis of model 2.The restricted cubic spline method was used to plot the dose-response relationship between GGT and creatinine levels and the risk for HUA.ResultsAmong 735 men, 192(26.12%) suffered from HUA.The HUA group and the non-HUA group had statistical differences in age, sleep, BMI,BMR,GGT,creatinine, TG, metabolic syndrome, coronary heart disease and incidence of stroke(P<0.05).The levels of GGT and creatinine were positively correlated with uric acid level, and the linear regression equation was statistically significant(P<0.05).Models 1,2,and 3 showed that compared with GGT<22 U/L,GGT 22 U/L-,30 U/L-and >44 U/L were the risk factors for HUA(P<0.05),and compared with creatinine<67 mmol/L,creatinine>81 mmol/L was the risk factor for HUA(P<0.01).Both creatinine and GGT levels had a positive linear dose-response relationship with the risk of HUA.GGT and creatinine had a certain interaction effect on the risk of HUA.When GGT and creatinine were both high(GGT>44 U/L,creatinine>81 mmol/L),the risk of HUA was the highest(P<0.01).Conclusion High levels of GGT and creatinine in male adults are the risk factors for the occurrence and development of HUA.
