益气固本通络方治疗糖尿病周围神经病变临床观察

摘　　要：目的探讨益气固本通络方在糖尿病周围神经病变患者中的临床疗效。目的选择2016年8月—2019年12月糖尿病周围神经病变患者84例为对象,随机数字表法分为两组各42例。对照组采用甲钴胺治疗,观察组在对照组基础上联合益气固本通络方治疗,治疗后评估患者效果并完成随访。采用肌电图仪器测定患者运动传导速度(MNCV)、感觉传导速度(SNCV)水平;采用酶联免疫吸附试验测定白细胞介素-1β(IL-1β)、可溶性血管细胞黏附分子-1(sVCAM-1)、同型半胱氨酸(Hcy)水平;采用全自动生化分析仪测定空腹血糖(FBG)、餐后2 h血糖(2 hPG)及糖化血红蛋白(HbAlc)、血清胱抑素C(Cys-C)水平,比较两组中医证候积分、运动传导速度、血糖水平、炎性因子、不良反应及复发率。结果观察组治疗后下肢麻木、肢体疼痛、感觉障碍、皮肤干燥、腰膝酸软评分均低于对照组(P<0.05);观察组治疗后SNCVV和MNCV中正中神经、尺神经和腓总神经速度均高于对照组(P<0.05);观察组治疗后FBG、2hPG及HbAlc、Cys-C水平均低于对照组(P<0.05);观察组治疗后IL-1β、sVCAM-1、Hcy水平均低于对照组(P<0.05);两组用药期间恶心呕吐、皮疹过敏、发热出汗发生率和治疗后1、6个月复发率无统计意义(P>0.05);观察组治疗后12个月复发率低于对照组(P<0.05)。结论益气固本通络方在糖尿病周围神经病变患者中能降低中医证候积分,提高患者运动传导速度,降低血糖及炎性因子水平,未增加不良反应发生率,能降低治疗后复发率,值得推广应用。Objective To investigate the clinical efficacy of Yiqi Guben Tongluo Decoration(益气固本通络方) in patients with diabetic peripheral neuropathy. Methods 84 patients with diabetic peripheral neuropathy from August 2016 to December 2019 were randomly divided into two groups, 42 cases in each group. The control group was treated with Mecobalamin, andthe observation group was treated with Yiqi Guben Tongluo Decoration on the basis of the control group. The effect of the patients was evaluated after 4 weeks of treatment and the follow-up was completed. The levels of motor conduction velocity( MNCV),sensory conduction velocity( SNCV),interleukin-1 β( IL-1β),soluble vascular cell adhesion molecule-1( s VCAM-1) and homocysteine( Hcy) were measured by enzyme-linked immunosorbent assay( ELISA);fasting blood glucose( FBG),postprandial blood glucose( 2 h PG) and glucose were measured by automatic biochemical analyzer The levels of Hb Alc and Cys-C were compared between the two groups. Results The scores of lower limb numbness,limb pain,sensory disturbance,skin dryness,lumbago and knee soreness and softness in the observation group were lower than those in the control group after treatment( P < 0. 05);the velocities of median nerve,ulnar nerve and common peroneal nerve in the observation group were higher than those in the control group after treatment( P < 0. 05);the levels of FBG,2 h PG,Hb Alc and Cys-C in the observation group were lower than those in the control group( P < 0. 05) The levels of IL-1 β,s VCAM-1 and Hcy in the observation group were lower than those in the control group after treatment( P < 0. 05);the incidence of nausea and vomiting,rash allergy,fever and sweating in the two groups were not statistically significant( P > 0. 05);the recurrence rate of the observation group at 12 months after treatment was lower than that of the control group( P < 0. 05). Conclusion Yiqi Guben Tongluo Decoction can reduce TCM syndrome score,improve motor conduction velocity,reduce blood glucose and inflammatory fact
