摘　　要：目的观察以Bobath理念指导的药棒疗法对脑卒中后手功能障碍的效果。方法选取符合入组标准的90例受试者,随机分为药棒组、Bobath组和改良药棒组,每组各30例,分别予药棒治疗、Bobath技术治疗和基于Bobath技术的中医药棒疗法治疗,观察治疗前后三组患者的上肢肢体痉挛程度、运动功能、手功能对日常生活能力等相关指标并进行评价分析。结果以Bobath理念指导的药棒疗法对脑卒中后上肢痉挛性瘫痪患者的Ashworth分级积分、上肢Fugl-Meyer运动功能评分的改善程度均优于单纯传统中医药棒技术和Bobath技术训练(P<0.05);以Bobath理念指导的药棒疗法对患者的UEFI汉化量表积分改善程度优于传统中医药棒疗法(P<0.05),与Bobath技术训练疗效相当(P>0.05)。结论以Bobath理念指导的药棒疗法对改善脑卒中后上肢痉挛、运动功能及手的日常活动功能的恢复有着满意的疗效。Objective To observe the effect of medicine stick therapy guided by Bobath concept on hand dysfunction after stroke.Methods 90 subjects were randomly divided into three groups: the medicine stick group, the Bobath group and the modified medicine stick group. Each group had 30 patients, who were treated with medicine stick, the Bobath technology and the traditional Chinese medicine stick based on the Bobath technology.Before and after treatment, the degree of upper limb spasm, motor function, hand function and daily life ability of the three groups were observed and evaluated. Results The improvement of Ashworth score and Fugl Meyer motor function score of upper extremity in patients with spastic paralysis after stroke was better than that of traditional Chinese medicine and Bobath training(P<0.05);The improvement of the scores of UEFI Chinese scale guided by Bobath concept was better than that of traditional Chinese medicine(P<0.05), which was equivalent to the effect of Bobath technical training(P>0.05).Conclusion The medicine stick therapy guided by Bobath concept has a satisfactory effect on the recovery of upper extremity spasm, motor function and hand daily activity function after stroke.
