万古霉素血药浓度与中粒细胞缺乏的相关性研究

摘　　要：目的通过分析血液肿瘤患者化疗后中性粒细胞缺乏(NP)与非NP者万古霉素谷浓度水平,探讨化疗后感染患者万古霉素的给药方案。方法用回顾性研究方法,收集北京大学第三医院血液科化疗后出现感染的60例血液肿瘤患者,分为NP组30例和非NP组30例。分析万古霉素血药浓度谷浓度水平与NP及其他因素的相关性。结果NP组和非NP组患者的万古霉素谷浓度分别为7.80(4.9013.10)和12.50(7.3016.50)mg·L^(-1),差异有统计学意义(P<0.05)。万古霉素谷浓度与患者是否为粒细胞缺乏及肌酸酐水平具有相关性。结论NP患者万古霉素谷浓度较低,对血液系统肿瘤化疗后患者使用万古霉素时需考虑患者是否为粒细胞缺乏状态。Objective To analyze the serum concentration of vancomycin in neutropenia(NP)and non-NP patients of hematological malignancy patients,and explore the optimal dosage of vancomycin in treating infection after induction chemotherapy.Methods This was a retrospective trial.Sixty patients,who got infection after receiving chemotherapy in department of hematology in Peking University Third Hospital,were involved in this trial.The study population consisted of 30 NP and 30 non-NP patients.The relationship between serum concentration of vancomycin and Neutropenia and other factors was analyzed.Results The serum tough concentration of vancomycin of NP and non-NP patients were 7.80(4.90-13.10)and 12.50(7.30-16.50)mg·L^(-1) with significant difference(P<0.05).Positive correlations were found between the serum tough concentration of vancomycin and neutropenia,creatinine levels.Conclusion The serum tough concentration of vancomycin was low,which should be taken into consideration when giving vancomycin in infective hematological malignancy patients after chemotherapy.
