新疆癫痫患儿拉莫三嗪稳态血药浓度影响因素的研究

摘　　要：目的探讨年龄、性别和合并用药对新疆癫痫患儿拉莫三嗪(LTG)稳态血药浓度的影响,并初步建立LTG稳态血药浓度参考范围,为其临床个体化用药提供理论依据。方法收集并测定266例规律服用LTG并达到稳态血药浓度的018岁新疆癫痫患儿血清药物浓度,并记录患儿年龄、性别、给药剂量及合并用药等基本信息,分析年龄、性别及合并用药对LTG血药浓度的影响,并初步建立新疆癫痫患儿LTG稳态血药浓度的参考范围。结果不同年龄、性别和族别癫痫患儿LTG的浓度剂量比(CDR)值差异无统计学意义(P>0.05)。5组患儿的LTG给药剂量和血药浓度均不相关(均P>0.05);LTG的CDR结果在不同合并用药组间的差异均无统计学意义(均P>0.05)。本研究总样本LTG稳态血药浓度参考区间为2.2016.24μg·mL^(-1)。结论儿科临床用药尤其合并用药时,有必要对LTG血药浓度进行监测并及时调整剂量,为新疆癫痫患儿制定个体化给药方案提供基础。Objective To explore the effects of age,sex and combined use of drugs on the steady-state blood concentration of lamotrigine(LTG)in children with epilepsy in Xinjiang,and to establish the reference range of steady-state plasma concentration of LTG,so as to provide theoretical basis for clinical individualized drug use.Methods The serum drug concentration was measured and collected in 266 children with epilepsy in Xinjiang who regularly took LTG and reached steady-state blood concentration,and the basic information such as age,sex,dosage and combination of drugs was recorded,and the effects of age,sex and combined use on the blood concentration of LTG were analyzed,and the reference range of steady-state blood concentration of LTG in children with epilepsy in Xinjiang was established.Results There was no significant difference in LTG of ratio of concentration dose(CDR)values among children with epilepsy of different ages,genders and ethnic groups(P>0.05).There was no correlation between LTG administration dose and blood concentration in five groups(all P>0.05).There were no statistically significant differences in CDR results of LTG among groups with different combinations of drugs(all P>0.05).The reference range of steady-state blood concentration of LTG in this study was 2.20-16.24μg·m L^(-1).Conclusion In pediatric clinical use,especially when combined with drugs,it is necessary to monitor the blood concentration of LTG and adjust the dose in time,so as to provide a basis for individualized drug administration for children with epilepsy in Xinjiang.
