氨甲环酸注射液用于老年半髋关节置换手术中出血患者的临床研究

摘　　要：目的观察氨甲环酸注射液用于老年半髋关节置换手术中出血的临床疗效及安全性。方法将316例患者按是否使用止血药分为对照组171例和试验组145例。对照组未给予止血药;试验组于术前10 min给予静脉注射15 mg·kg^(-1)氨甲环酸。比较2组患者的输血率,总失血量,术后第1和3天的血红蛋白(Hb)水平,以及不良事件的发生情况。结果试验组和对照组的输血率分别为7.59%(11例/145例)和26.90%(46例/171例),总失血量分别为(488.16±87.25)和(574.58±86.50)mL,术后第1天Hb分别为(105.45±5.45)和(96.55±5.49)g·L^(-1),术后第3天Hb分别为(96.27±5.51)和(91.36±6.26)g·L^(-1),差异均有统计学意义(均P<0.05)。2组患者的不良事件均以深静脉血栓为主。试验组和对照组的总不良事件发生率分别为0.69%和1.75%,差异无统计学意义(P>0.05)。结论氨甲环酸注射液可以显著减少老年半髋关节置换手术中出血,降低输血风险,且安全性较高。Objective To observe the clinical efficacy and safety of tranexamic acid injection in the treatment of bleeding in geriatric hemiarthroplasty surgery.Methods A total of 316 patients were divided into control group(n=171 cases)and treatment group(n=145 cases)according to whether to use hemostatic or not.The control group was not given hemostatic.Treatment group was given 15 mg·kg^(-1) tranexamic acid intravenously 10 min before operation.The transfusion rate,total blood loss,hemoglobin(Hb)levels on postoperative day 1 and 3,and the adverse events were compared between two groups.Results The transfusion rates of treatment and control groups were 7.59%(11 cases/145 cases)and 26.90%(46 cases/171 cases),total blood loss was(488.16±87.25)and(574.58±86.50)mL,the levels of Hb on postoperative day 1 were(105.45±5.45)and(96.55±5.49)g·L^(-1),the levels of Hb on postoperative day 3 were(96.27±5.51)and(91.36±6.26)g·L^(-1),the differences were statistically significant(all P<0.05).The adverse events of two groups were deep venous thrombosis.The incidences of total adverse events of treatment and control groups were 0.69%and 1.75%without significant difference(P>0.05).Conclusion Tranexamic acid injection can significantly reduce bleeding and decrease the risk of blood transfusion during hemiarthroplasty surgery in the elderly with a high safety.
