超高龄肾功能不全心房颤动患者抗凝治疗的病例分析

摘　　要：临床药师通过参与1例心房颤动合并肾功能不全的超高龄老年患者药物治疗过程,综合考虑其出血危险因素,依据抗Xa活性浓度为患者制订个体化抗凝治疗方案。在临床药师的参与治疗下,患者多次调整抗凝药物,凝血功能监测在安全范围,无出血和卒中事件发生,好转出院。Pharmacist participated in the treatment process of one super-aged patient with renal insufficiency and atrial fibrillation,and formulated an individualiazed anticoagulant therapy by comprehensively considering bleeding risk and anti-factor Xa activity.With the participation of pharmacist and integration of services,the patient adjusted anticoagulants properly and coagulation function was under a safety range.The patient was in a better condition and discharged with no bleeding or stoke events occurred.
