摘  要:肾脏作为血液滤过、物质交换及调节体内水平衡等多方面功能的复杂性器官,临床上多种功能监测方法均存在一定局限性。肾移植术后患者移植肾通常面临失功能的风险,早期发现移植肾功能受损可早期进行对症治疗从而为移植肾术后维持肾功能提供更多的帮助。功能性磁共振成像(fMRI)技术近年来的应用发展迅速,并在移植肾功能监测、随访中发挥重要作用。fMRI(DWI、IVIM、DTI、DKI、BOLD、ASL)是一类利用磁共振成像对组织微观结构及化学成分进行描述和测量的新技术,可在无创无辐射无对比剂引入的条件下获得肾微观结构变化信息。fMRI利用水分子扩散、微循环扩散、血氧分布、血流灌注等信息在监测肾功能的潜在变化及随访评估等方面提供更多价值。本文就fMRI的发展现状及其在肾脏和移植肾功能监测、随访等方面的研究进展进行了综述。The renal is a complex organ with many functions,such as hemofiltration,substance exchange and water balance regulation.There are some limitations in clinical monitoring of renal function.The patients after renal transplantation usually need to face the risk of renal dysfunction.Early detection of renal allograft dysfunction can provide early symptomatic treatment,so as to provide more help for the maintenance of renal function after renal transplantation.In recent years,the applitation of functional magnetic resonance imaging(fMRI)technology has developed rapidly,and it plays an important role in the monitoring of renal allograft function and follow-up.fMRI(DWI,IVIM,DTI,DKI,BOLD,ASL)is a kind of new technology that uses magnetic resonance imaging to describe and measure the microstructure and chemical composition of tissues,and can obtain the information of renal microstructure without invastion,radiation and contrast agent.f MRI can provide more value in monitoring potential changes of renal function and follow-up evaluation by using information of water molecular diffusion,microcirculation diffusion,blood oxygen distribution and blood perfusion.This article reviews the development of fMRI and its progress in monitoring and follow-up of renal and renal allograft function.
