基于分级用药理论的沈氏女科利水渗湿药应用分析

摘　　要：沈氏女科在其650余年的临床实践中积累了大量的用药技巧,"分级用药"便是其用药特色之一。笔者梳理利水渗湿药物文献,结合现代临床与中药药理学研究,对利水渗湿药物的作用强弱做出轻中重三级的分类。分别梳理汉代、晋唐、宋元、明清4个时期利水渗湿药物的分级应用,并介绍沈氏女科利水渗湿药物、常用方剂的分级及应用。首次系统揭示了利水渗湿药物的三级应用模式,并构建利水渗湿药"识别-定级-间接治疗"的决策模式。以期为沈氏女科三级应用的临床推广提供理论基础,且弥补辨证论治定量研究的不足,为辨证论治定量研究提示一个新的研究方向。Shen’s gynecology has accumulated a lot of medication skills in its 650 years of clinical practice,and"graded medication"is one of its medication characteristics.Through combing the literature of diuresis-inducing and dampness-draining drugs,combining modern clinical and traditional Chinese medicine pharmacology research,the drugs are divided into three levels as follows:light,medium and heavy according to their strength.This paper reviews the classification and application of these drugs in Han Dynasty,Jin Tang Dynasty,Song Yuan Dynasty,Ming and Qing Dynasty,and introduces the classification and application of Shen’s gynecology’s rehydration drugs and commonly used prescriptions.It is the first time to systematically reveal the three-level application mode of these drugs,and to construct the decision-making mode of"identification,classification and indirect treatment"of these drugs.This paper aims to provide a theoretical basis for the clinical application of the three-level application of Shen’s gynecology,make up for the deficiency of the quantitative research of syndrome differentiation and treatment,and provide a new research direction for the quantitative research of syndrome differentiation and treatment.
