补肾祛瘀汤合并三棱针治疗足踝部废用性骨质疏松症临床观察

摘　　要：目的观察补肾祛瘀汤合并三棱针穿刺放血治疗偏瘫后继发足踝部废用性骨质疏松症的临床疗效。方法选择66例偏瘫后继发足踝部废用性骨质疏松症的患者,随机分为观察组与对照组,观察组采用口服补肾祛瘀汤,三棱针在足踝部穴位穿刺放血,同时进行康复锻炼;对照组采用口服钙尔奇D,同时进行康复锻炼,治疗2、4、6周时采用疼痛视觉模拟评分法(VAS)评定行走痛、休息痛的疼痛程度。治疗6周后观察临床治疗效果。结果经统计学处理,2组总有效率差异有统计学意义:2组患者治疗2、4、6周观察组患者的休息痛改善优于对照组(P<0.05),治疗4、6周后观察组患者的行走痛改善优于对照组(P<0.05),观察组总有效率优于对照组(P<0.05)。结论三棱针穴位穿刺放血能有效缓解患者的休息痛和行走痛,配合补肾活血中药治疗偏瘫后继发足踝部废用性骨质疏松症,临床疗效确切,值得推广应用。Objective To observe the clinical effect of Bushen Quyu decoction combined with three-edged needle therapy and bloodletting in the treatment of secondary disused osteoporosis of foot and ankle after hemiplegia.Methods 66 patients with disuse osteoporosis of foot and ankle after hemiplegia were randomly divided into the observation group and the control group.The observation group was treated with oral Bushen Quyu decoction combined with three-edged needle therapy and bloodletting,and rehabilitation exercise was performed at the same time.The control group was treated with oral Caltech D,at the same time,rehabilitation exercise was carried out.VAS score was used to evaluate the pain degree of each patient after rest and walking.The clinical effect was observed after 6 weeks.Results After statistical treatment,the total effective rate of the two groups was significantly different.The rest pain of the two groups was better than that of the control group(P<0.05),the walking pain of the observation group was better than that of the control group(P<0.05),and the total effective rate of the observation group(%)was better than that of the control group(P<0.05).Conclusion Three-edged needle therapy and bloodletting can effectively relieve the rest pain and walking pain of the patients.Combined with traditional Chinese medicine of invigorating kidney and activating blood circulation,it can treat the secondary disuse osteoporosis of foot and ankle after hemiplegia,which has a definite clinical effect and is worth popularizing.
